- Statement of Work THIS IS A NOTICE OF INTENT TO AWARD A SOLE SOURCE CONTRACT AND IS NOT A REQUEST FOR COMPETITIVE QUOTES. Reference #: N0025917N0067. Naval Medical Center San Diego intends to award a firm fixed price sole source contract under the authority of FAR 13.106-1(b)(1)(i), only one responsible source, to: Best Practices Professionals, Inc.; 1475 Military Way.; Salt Lake City, UT, 84103-4457. The objective of this sole source award is for a one year contract (to include two option years) Document Site online subscription service. Salient Characteristics must include: 1. annual access to all Instruction For Use (IFU) and Preventative Maintenance (PM) 2. Compliance with CMS, Joint Commission mandates for hospitals, AAAHC, AAMI accreditation standards 3. Able to add requested documents or manuals to equipment database at no extra charge Base Year: Period of Performance 12 MAY 2017 - 11 MAY 2018 CLIN 0001: Equipment, Biomed and Surgical Instrument Database Access Subscription, Quantity: 12; Unit of Issue: Monthly. Includes requirements as stated in the Statement of Work. CLIN 0002: Dental Database Access Subscription; Quantity: 12; Unit of Issue: Monthly. Includes requirements as stated in the Statement of Work. Option Year One: Period of Performance 12 MAY 2018 - 11 MAY 2019 CLIN 0001: Equipment, Biomed and Surgical Instrument Database Access Subscription, Quantity: 12; Unit of Issue: Monthly. Includes requirements as stated in the Statement of Work. CLIN 0002: Dental Database Access Subscription; Quantity: 12; Unit of Issue: Monthly. Includes requirements as stated in the Statement of Work. Option Year Two: Period of Performance 12 MAY 2019 - 11 MAY 2020 CLIN 0001: Equipment, Biomed and Surgical Instrument Database Access Subscription, Quantity: 12; Unit of Issue: Monthly. Includes requirements as stated in the Statement of Work. CLIN 0002: Dental Database Access Subscription; Quantity: 12; Unit of Issue: Monthly. Includes requirements as stated in the Statement of Work. Best Practices Professionals, Inc. is the sole distributor of the needed items. The NAICS code for this requirement is 519130; the small business size standard is 1,000 employees. This notice of intent is not a request for competitive quotes; however, interested persons may identify their interest and capability to respond to this requirement. All information received by 3:00 PM Pacific Time on May 03, 2017 may be considered by the Government. Responses shall be submitted to Gilberto Esteves at gilberto.a.esteves.civ@mail.mil. The email subject line shall state, "Naval Medical Center San Diego, Intent to Award Sole Source to Depuy Synthes Sales, Inc."
