Description

Purpose and Objectives: The intent of this Sources Sought Notice is to identify potential offerors capable of providing Vest Airway Clearance System with Stand as listed below in the Statement of Need. These are for the Lebanon Veteran Affairs Medical Center in Lebanon, PA 17042. Responses to this Sources Sought Notice should demonstrate the firm's ability, capability, and responsibility to provide the principal components of supplies listed in the attached document. Responses should include the following information: Business name, address, Point of Contact, Service Disabled Veteran Owned Small Business info. All information is to be submitted via e-mail at Daniel.Johnson44@va.gov. Information provided will not be returned. All responses shall be in the English Language. Responses are due by 1200 pm (EST) on Monday, May 1, 2017. No submissions will be accepted after this date and time. Questions can be submitted electronically to at Daniel.Johnson44@va.gov. This is a Sources Sought Notice and submissions will be used for informational and planning purposes only. This notice does not constitute a formal Request for Quote (RFQ), nor is the government obligated to issue an RFQ. In addition, the Government does not intend to pay for any information provided under this notice. The Government is not obligated to notify respondents of the results of this survey. STATEMENT OF NEED: Lebanon VA Medical Center has a requirement for the following Vest Airway Clearance System with Stand; one (1) Vest System including Control Unit with Cart, one (1) Vest Garment including Slotted Vest Wrap SPU-Medium, one (1) Vest Garment including Slotted Vest Wrap SPU-Large. The Lebanon VA Medical Center (595) has a need for the listed equipment items to be delivered to the warehouse located at Building 6 or 1700 S. Lincoln Ave, Lebanon, PA 17042. PRODUCT SCHEDULE: CLIN #1 Vest System Control Unit with Cart Must have a simple, flexible programming -Normal Mode -Ramp Mode -Program Mode with Cough Pause ® Option Must ease off pressure when paused Must have an easy-to-read screen Must ease into therapy Must have optional remote control Must have optional disposable air hoses Must have easy movement on four independent, swiveling casters Must be convenient and have quick-access storage for disposable garments Must have Push-buttons for data entry Must have dual language capability Must have small footprint for easy positioning and storage Must have adjustable height to offer variable positioning at the bedside, chairside, or during transport Must have disposable vests in multiple sizes 1 EA CLIN #2 Vest Garment Slotted Vest Wrap SPU-Medium 1 EA CLIN #3 Vest Garment Slotted Vest Wrap SPU-Large 1 EA
