Description
Bid schedule Solicitation This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. Solicitation No. W912EE-17-T-0024 is being issued as a Request for Quote (RFQ). This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-82. This procurement is 100% set-aside for Small Business. The North American Industry Classification System Code (NAICS) for this procurement is 621999, with Size Standard of $15.0 M. The U.S. Army Corps of Engineers (USACE), Vicksburg, Mississippi, Vicksburg District, Mat Sinking Unit has a requirement for Advanced Life Support (ALS) services, equipment and personnel of two (2) Licensed Paramedics. This contract will consist of a base season along with four option seasons. If the Mat Sinking Unit determines that contractor's performance is acceptable during the base season, the first option season will be executed for the next mat sinking season. And so on until all option seasons are utilized or until the contractor delivers unsatisfactory services. BID SCHEDULE: Reference the Bid Schedule posted within this solicitation and as a separate attachment under Additional Documentation. This MUST be returned with your quote along with the SF 1449 (complete box 30a, 30b, and 30c) and all other requested documentation. GENERAL PURPOSE: The purpose of this contract is to provide Advanced Life Support (ALS) & Basic Life Support (BLS) services, treatment protocols, medical direction, equipment and personnel of (2) Paramedics. This includes all supplies, equipment, drugs, treatment protocols and medical direction needed to provide BLS and ALS treatment. The contractor will be required to provide health care services for the employees of the U.S. Army, Corps of Engineers, Mat Sinking Unit. The services of this contract will be for a period of performance of 6 months; based upon the length of the work season. The period of performance will begin on or about June 5th 2017. SPECIFICATIONS: Reference the Specifications and Bid Schedule (posted within the solicitation and as a separate attachment) posted under Additional Documentation. SAM Registration: All offerors shall be registered in the System for Award Management (SAM), if you are not registered in (SAM), an award CANNOT be made to your company. You may register electronically at http://www.sam.gov. Quotes are due 10 May 2017, not later than 12:00 p.m. Central Time at the USACE Vicksburg District Contracting Office, 4155 East Clay Street, Vicksburg, MS, 39183. Quotes should be sent to the attention of Jason M. Carlock. Quotes may be submitted via email to Jason.M.Carlock@usace.army.mil provided all required information is included.
