Description

(i)This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. (ii) Solicitation number VA261-17-Q-0371 is issued as a request for quote (RFQ). (iii) The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-94 and 2005-95. (iv) This acquisition is a 100% Small Business set aside. The NAICS code is 624229 and the size standard is $15 million. (v) The contract line items are listed in the price schedule. When providing a quote, please provide a fixed monthly rate for each line item along with the total for the 12 months. (vi) The contractor shall provide housing placement and case management services through the US Department of Housing and Urban Development Veterans Affairs Supportive Housing(HUD-VASH) program for the San Francisco VA Health Care System Humboldt County (Eureka) in accordance with the attached Statement of Work. (vii) The contract will be for a base year plus one option year. The period of performance is listed in Section B of the solicitation. (viii) The provision 52.212-1, Instructions to Offerors Commercial Items and addendum with additional instructions/information, applies to this acquisition. (ix) The provision 52.212-2, Evaluation Commercial Items and addendum with additional instructions/information, applies to this acquisition. (x) The provision 52.212-3, Offeror Representations and Certifications Commercial Items, applies to this acquisition. If a contractor has already provided this information in their System for Award Management (SAM) profile, then no additional information is needed under this provision. (xi) The clause 52.212-4, Contract Terms and Conditions Commercial Items, applies to this acquisition. (xii) The clause 52.212-5, Contract Terms and Conditions Required To Implement Statutes or Executive Orders Commercial Items, applies to this acquisition. The clauses checked within clause 52.212-5 in Solicitation Number VA261-17-Q-0371 are applicable to this acquisition. (xiii) The provisions and clauses included within solicitation VA261-17-Q-0371 are applicable to this Acquisition. (xiv) The Defense Priorities and Allocations System (DPAS) do not apply to this acquisition. (xv) Quotes are due by 1500 or 3 PM PST on May 29, 2017 and can be emailed to angela.gerlitzki@va.gov.
