Description
Instructions to offerors The Department of Homeland Security, Office of Health Affairs (OHA), is hereby requesting that vendor's submit an offer to provide services to be furnished in accordance with the attached Statement of Work (SOW) entitled "Hazardous Waste Disposal for the Illinois Department of Public Health (IDPH) laboratory". Title: Hazardous Waste Disposal RFQ No.:HSHQDC-17-Q-00109 Quote Due Date:On or Before Wednesday May 3, 2017 at 2:00 PM EST Purchase Order Type:Firm-Fixed-Price Statement of Work:See Attachment 2 to RFQ Pricing Schedule:See Attachment 3 to RFQ Period of Performance:12-month Base period and four 12-month Option periods Points of Contact:Danielle H. Richardson, Contracting Officer Danielle.richardson1@hq.dhs.gov; and Sid Routh, Contract Specialist Sid.routh@hq.dhs.gov GENERAL INSTRUCTIONS: This section specifies the format and content that shall be used for quotations in response to this RFQ. Offerors are required to respond in accordance with these instructions. The goal is to ensure a certain degree of uniformity in the format of the responses for evaluation purposes. In order to receive full consideration, Offerors are encouraged to ensure that the information provided within their quote is factual, accurate and complete. The Offeror is responsible for providing adequate documentation to enable a thorough evaluation of the quote. The Offeror shall submit a quote that clearly and concisely describes and defines the Offeror's response to the Statement of Work (SOW) contained in the RFQ. Unnecessary elaboration or other presentations beyond that sufficient to present a complete and effective quote are not desired. Hasty responses or responses which merely repeat the SOW may result in a marginal or unacceptable rating. Assurances of experience, capability, qualifications, without a clear demonstration to support this claim, will adversely influence the evaluation of the quote. The quote shall contain all the pertinent information in sufficient detail in the area of the quote where it contributes most critically to the discussion of the same information. When necessary, the Offeror shall refer to the initial discussion and identify its location within the quote. Offerors should also identify risks inherent in their quote, techniques used to mitigate those risks, and where such techniques were successfully employed in other contracts. If an Offeror does not understand the instructions contained herein, written notification should be provided via email to the Contracting Officer via the Contract Specialist for clarification sufficiently in advance of the deadline for the receipt of quotations to get an answer in time to meet the deadline. The Government will distribute the questions asked and the answers given them to all prospective Vendors. Alternate quotes are not authorized. Exceptions to any terms and conditions of the RFQ may make the offer unacceptable. Quotations must be submitted electronically in a format readable by Microsoft (MS) Word 2010, MS Excel 2010, or Adobe Portable Document Format (PDF). Quotes must be submitted electronically (via e-mail) to Danielle H. Richardson, Contracting Officer, and Sid Routh, Contract Specialist via the email addresses listed above. The e-mail subject line must specify the "HSHQDC-17-Q-00109 - Hazardous Waste Disposal, and the name of the Offeror". Facsimile responses will not be accepted. Quotations shall be submitted using WinZip software. The WinZip password shall be submitted under a separate email by the due date. Please note: There is not a character length or mixed case requirement imposed on your password. The password can be a simple password but should not be a common word. These submission instructions will also apply to any future correspondence, as applicable, in response to this solicitation. Pages shall have at least one inch margins on all sides, using Times New Roman font, with 12 point font size for all body text. Text used in graphics is at the discretion of the Vendor, but should be a legible font and size. Graphics shall count towards page limitations. Vendors shall not submit information in their quotes that is considered "CLASSIFIED". Proprietary markings are permitted. Pricing information shall be provided only in the Price Quote (Volume II). To verify receipt of your response, the contract specialist will send an email confirming receipt. Questions or assumptions regarding this solicitation must be submitted in writing via e-mail to danielle.richardson1@hq.dhs.gov and sid.routh@hq.dhs.gov no later than Monday, May 1, 2017, at 10:00 AM Eastern Time. Questions submitted to any other point of contact will not be accepted. Questions should be submitted in the following format: Document (SOW/pricing table/1449)Page #Question Upon receipt and evaluation of quotes the Government intends to make an award without further discussion; however, this does not prohibit the Government from holding discussions prior to award if the contracting officer determines discussions to be in the best interest of the Government. Vendors are required to submit their quotes in separate volumes as indicated below. There shall be no reference to pricing included as part of the Technical Volume. Volume I :Cover Letter and Technical Quote Volume Volume II: Price Volume Each volume shall contain the following items in addition to the technical /descriptive data required by the RFQ: Cover Letter: The cover letter should indicate the following: Title of the quote Quote volume Volume No. Name and address of Offeror Name and phone number of a principal point of contact Table of Contents:The table of contents shall provide sufficient detail as to allow the important elements to be easily located. 1.0VOLUME I - COVER LETTER/TECHNICAL QUOTE VOLUME Tab A: The Cover Letter shall not exceed one (1) page. The cover letter must contain a statement committing the Offeror to the quote and stating its validity for a period of at least 90 days. This letter should also include the name, title, phone number, and e-mail address for the persons authorized to discuss the quotation with the Government. The letter must be signed by a responsible officer or partner/principal of the Vendor Team Leader that is authorized to bind the organization contractually. Tab B: The Technical Quote shall not exceed three (3) pages (not including past performance information). NOTE: While the technical quote must not contain any reference to cost, resource information (such as data concerning labor hours and categories, materials, subcontracts, etc.) must be provided so that the contractor's understanding of the requirements may be evaluated. TAB B1 - Factor 1: Technical Approach - The offeror shall demonstrate their capability and approach on their: •Understanding of the work, including creativity and thoroughness shown in understanding the objectives of the SOW and specific tasks, and planned execution of the project. Understanding of the work includes identifying the applicable Federal/State/Local regulations to the work described. •Evidence of specific methods and techniques for completing each discrete task, to include such items as quality assurance and customer service. Tab B2 - Factor 2: Past Performance Past performance information may cite federal, state, local government, commercial sources, or not for profit sources and should highlight the company's key role in the past performance being documented. This information shall consist of a one (1) page introductory matrix listing the offeror's and its critical subcontractors' past performance references. The offeror shall provide a minimum of three (3) references. In addition, the offeror may submit a minimum of three (3) references for subcontractors who are expected to perform aspects of this effort that the offeror considers critical to overall successful performance. The offeror should include rationale supporting the assertion of relevance for each reference. These references should also be from different contracts. To be considered, past performance data may not be older than three (3) years from the date of the solicitation, and may include active contracts. Further, offerors should include a list of all relevant DHS on-going contracts or contracts completed for the specified period. Past performance references shall be as a prime contractor or first-tier subcontractor on previous or current projects/tasks that are similar in size, scope, and complexity to the work identified in the Statement of Work (SOW). Past performance does not necessarily have to be Government-related; but it is important to demonstrate and provide evidence of past performance for work similar in size, scope and complexity to that described in the Request for Quotation (RFQ). The offeror is required to identify any performance problems/issues/challenges arising out of the contract and clearly describe management actions implemented to overcome problems and the effects of those actions, in terms of improvements achieved or problems solved. Evidence of performance-based methodologies in past performance shall be provided where available. It is the offeror's responsibility to notify all references that they will be contacted by the contracting officer and ensure that they make themselves available to the contracting officer. The contracting officer will make a good faith effort to contact the references. If after making a good faith effort, the contracting officer is unable to contact a reference without causing undue delay to the procurement process, the contracting officer will document the efforts made to contact the reference. Non-responses from references may not be evaluated favorably or unfavorably on the past performance evaluation. 5. The Government reserves the right to use publicly available reports and data from the Past Performance Information Retrieval System (PPIRS) found on the web at http://www.ppirs.gov. The Government may also use present and/or past performance data obtained from a variety of sources, not just those contracts identified by offerors. For each reference, provide: a.Reference Name (Company or Entity) b.Program Title c.Contracting Agency/Private Company d.Contract Number/Task Order Number (if applicable) e.Brief description of the contract effort, including results and successes f.Type of Contract g.Period of Performance h.Original contract dollar value and current or actual dollar value i.Actual completion date j.Contact names, addresses, telephone numbers, Fax numbers and e-mail addresses of current, or last (if contract completed) Program Director/Manager and/or Contract Manager. 2.0VOLUME II - PRICE VOLUME Factor 3 - Price The Offeror shall submit a Firm-Fixed Price (FFP) quotation. The Offeror shall provide an adequate description and/or build-up of how they arrived at the proposed FFP. Price may be broken down by months. The Offeror shall complete the pricing template provided in the RFQ. The price of each year shall be distinguishable from the total evaluated price. (See Attachment 3: Pricing Template) If needed, the Government intends to exercise the option or options under FAR 52.217-8 without further competition or need for limited source justification for other than full and open competition. The Government will validate that any information provided in other parts of the quotation is consistent with the information provided in the Price Quotation. Any inconsistencies will be noted and may adversely affect the Vendor's evaluation rating. General Information about Evaluation Award shall be made to the offeror with the lowest price technically acceptable quote. Technical Evaluation: The Government will conduct a detailed evaluation of the offeror's approach and capability to meet the Government's objectives against the following technical evaluation factors: Factor 1 - Technical Approach •The government will evaluate the offerors quotation to determine the extent of the offerors understanding of the work, including creativity and thoroughness shown in understanding the objectives of the SOW and specific tasks, and planned execution of the project. The government will evaluate the offerors quotation to the extent of their understanding of the work to include them identifying the applicable Federal/State/Local regulations to the work described. •The government will evaluate the offerors quotations to determine the extent of the offerors demonstration of their evidence of specific methods and techniques for completing each discrete task, to include such items as quality assurance and customer service. Factor 2 - Past Performance The offeror's proposal will be evaluated to determine the extent to which their past performance demonstrates the likelihood of successful performance in providing requirements similar in size, scope and complexity of this solicitation. The evaluation will consider both the relevance and quality of past projects. The Government may choose to Retrieval System (PPIRS). The Government may use present and/or past performance data obtained from a variety of sources, not just those contracts identified by the offeror. The information gathered for past performance may also be used in the responsibility determination. The Government will review all past performance data submitted with the proposal and will contact all of the referenced companies/organizations and others to verify past performance information. Price Evaluation: Factor 3 - Price The Government will evaluate offeror's individual and total summary prices proposed in the Price Quote Spreadsheet. Quoted prices will be reviewed and analyzed in depth to determine if they are fair, reasonable and realistic for the services proposed, reflect a clear understanding of the requirements, and is consistent with the methods of performance described in the Offeror's quote. The Government will evaluate Quotations for award purposes by adding the total price (including base period, option periods, including the option to extend service under FAR 52.217-8, and optional tasks) for this requirement. Since the optional tasks are repeated in the base period and the two option periods they will only be evaluated once at the highest proposed price for each optional task. Evaluation of the option periods and optional tasks does not obligate the Government to exercise the options. If needed, the Government intends to exercise the option or options under FAR 52.217-8 without further competition or need for limited source justification for other than full and open competition. For purposes of evaluation, the potential need to exercise the option under FAR 52.217-8 to extend the period of task order performance for the maximum period of six (6) months beyond the last option period will be considered the same for all vendors. In considering the price of the base period and any option periods, the Government will consider that if the extension of service clause FAR 52.217-8 is exercised, it will be at the exact same price/rates and terms, other than length of performance, as the base or option period being extended. The Government will determine whether the price, inclusive of all options (including the options available under FAR 52.217-8), is fair and reasonable, and whether the price of the base period and all option periods (including the options represented by FAR 52.217-8), in combination with the other evaluation factors specified in the solicitation, represents the best value to the Government.
