Description

Hazardous and Non-Hazardous Medical Waste Removal VA Medical Center Salt Lake City, UT 84148 BRIEF SCOPE OF WORK (This is for information only) Project Scope 1. Contractor is to provide all labor and materials to pick up, transport and dispose of RCRA hazardous and RCRA nonhazardous waste, in accordance with the following requirements: a. Provide waste profiling services on an as needed basis. b. Provide and attach Department of Transportation (DOT) markings and labels to each container. c. Prepare shipping documentation using EPA ID UT9360090048. d. Verify integrity of all containers. e. Provide pick up and disposal services on a monthly basis. f. Provide certificates of destruction for RCRA hazardous waste. g. Waste is to be picked up at the Building 14 dock, Building 38, Building 1 dock, Building 14 Consolidated Hazardous Waste Accumulation Point (CHAP) and Building 2. 2. Contractor is to provide all labor and materials to pick up, transport and dispose of RCRA regulated hazardous chemotherapy waste and nonhazardous chemotherapy waste, in accordance with the following requirements: a. Provide waste profiling services on an as needed basis. b. Provide and attach Department of Transportation (DOT) markings and labels to each container as required. c. Prepare shipping documentation using EPA ID UT9360090048 for RCRA regulated medical waste. d. Verify integrity of all containers. e. Provide pick up and disposal services on a monthly basis. f. Provide certificates of destruction for RCRA hazardous waste. g. Waste is to be picked up at the Building 14 dock, and room BC05-14.
