Description
This is Sources Sought announcement only this is not a Request for Quote. The purpose of this notice is to conduct market research and obtain information from qualified vendors with special interest. No solicitation is being issued at this time. This notice shall not be construed as a commitment by the Government to ultimately award a contract, nor does it restrict the Government to a particular acquisition approach. All information submitted in response to this announcement is voluntary; the Government will not pay for information requested nor will it compensate any respondent for any cost incurred in developing information provided to the Government. The U.S. Department of Veterans Affairs, Network Contracting Office 2, and the VA Medical Center in Buffalo, NY anticipate a purchase order for the following: CR BARD Site~Rite 8 Ultrasound (1 each of all lines) 9770501 Ultraound with Pinpoint GT Probe 9770131 Sherlock 3CG TCS Sensor 9770116 MER Stand 9770509 SR8 Dicom software Key 9770508 SR8 Diamond software key (3CG) 9770083 Brother Printer 9770517 Site~Rite 8 Keyboard 9770401 Training Phantom Characteristics Vendor must provide a like or equal (when compared to the Bard Site~Rite 8 Ultrasound System) Pic Line Ultrasound System to the VAWNY. The device must be able to provide live vascular ultrasound images in order to allow staff to place PICC lines in patients. The device must be on a portable roll stand. The device must be compatible with the VA Electronic Patient Record System (VISTA/CPRS) and must have a VA approved DICOM Conformance Statement. The device must include a printer. The vendor must come on-site to assemble all parts of the device once it is delivered to the VAWNY. The vendor must install the device in a location determined by the VAWNY. The vendor must provide clinical user training on-site to any/all staff who request training. At least one site visit lasting a minimum of eight hours is required in order to complete training. The vendor must provide a minimum one year warranty on all products purchased. The appropriate NAICS Code is 339112 - Surgical and Medical Instruments This procurement is for new items only; no remanufactured or "gray market" items. All equipment must be covered by the manufacturer's warranty. FOB - destination Interested vendors are reminded that in accordance with FAR 4.12, prospective contractors shall complete electronic annual representations and certifications in conjunction with FAR 4.11 required registration in the System for Award Management (SAM) database prior to award of a contract. Responses should address how you will meet the Government's needs and provide a brief capability statement.
