- Sources Sought Information Request Form Draft PWS This is not a solicitation announcement. This is a sources sought synopsis only. The purpose of this synopsis is to gain knowledge of potential qualified sources and their size classification (Hubzone, 8(a), small, small disadvantage, service disabled veteran owned small business, etc.) relative to NAICS 621999, Ambulatory Health Care Services (size standard of $15 Million). Responses to this synopsis will be used by the Government to make appropriate acquisition decisions. After review of the responses to this sources sought synopsis, a solicitation announcement may be published on the FedBizOpps website. Responses to this sources sought synopsis are not considered adequate responses to the solicitation announcement. All interested offerors will have to respond to the solicitation announcement in addition to responding to this sources sought. No reimbursement will be made for any costs associated with providing information in response to this synopsis or any follow up information requests. The Florida National Guard, Immigration and Customs Enforcement, Office of the State Surgeon, is seeking potential Contractors capable of performing all functions within the "Draft" Statement of Work for Occupational Medical Surveillance Examination Services. Performance will take place in various locations throughout Florida during a two (2) week period. Contractors with the expertise to successfully perform the requirements outlined in the "Draft" Statement of Work attached are encouraged to submit a capability statement. If you are interested, and are capable of performing, please provide the requested information as indicated below (not to exceed five (5) pages): (1) Positive statement of intent to bid/submit proposal as a prime contractor; (2) A completed and signed Sources Sought Information Request Form (attached); and (3) A listing of service contracts (minimum 3 references) completed during the past three years, both for government and private industry (type of project, dollar value, contract number, location, and Point of Contact are to be included). Indicate if you were the prime or the subcontractor. If you were the subcontractor, then provide the name and point of contact for the prime contractor. Responses to this notice shall be e-mailed to the attention of Jeff Moore, at the following e-mail address: Jeffrey.d.moore14.mil@mail.mil. Telephone responses will not be accepted. Questions and/ or Responses must be received in no later than 4:00 p.m. EDT, Tuesday, May 9, 2017. This notice is to assist with determining sources only. A solicitation is not currently available. If a solicitation is issued it will be announced at a later date, and all interested parties must respond to that solicitation announcement separately from the responses to this announcement. A response to this sources sought is not a request to be added to a prospective bidders list or to receive a copy of the solicitation.
