Description

The Department of Veterans Affairs, Central Alabama Veterans Health Care System (CAVHCS), 215 Perry Hill Road, Montgomery, Alabama is seeking sources to provide outpatient services in a private hospital, office or clinic environment to veterans primarily residing in the Dothan, Alabama area. The Contractor facility must be physically located within the city of Dothan, AL which is approximately 107 miles from parent facility. The estimated number of patients enrolled/assigned to site is 4200. The applicable NAICS code is 621498 All Other Outpatient Care Centers (size standard $20.5 M) POTENTIAL SOURCES SHALL PROVIDE THE FOLLOWING INFORMATION IN THEIR RESPONSE: Company name, address, phone number, primary contact(s), e-mail address, DUNS number, and business type, e.g. Service Disabled Veteran Owned Business, Woman Owned Small Business, etc. as defined by the applicable NAICS. A statement of capability and relevant documentation demonstrating successful past performance in providing the aforementioned services (10 page limit).
