Description

Issuing sources sought for veteran businesses (SDVOSB/VOSB) and any small businesses dated 5/1/2017. The veteran business must be registered in SAM and certified in VETBIZ (VIP). The small businesses must be registered in SAM. VA Butler Healthcare (529) has a need for software compatible with current EMG system. The software will allow interface directly with a patient s Computerized Medical Record, significantly reducing the amount of time a clinician spends on duplicate documentation. VA Butler currently has an EMG system, which is in good condition, but the providers must manually document the test results. This item should be delivered to VA Butler Healthcare at 325 New Castle Road, Butler PA 16001, room # 306-East on Thursday, July 6, 2017. BACKGROUND Cadwell Laboratories, Inc. is the sole manufacturer, distributor, and service provider of all Sierra EMG systems & the HL7 interface. VA Butler purchased the Sierra Wave in November, 2013. The HL7 interface allows the Sierra Wave to receive order messages from the Electronic Medical Record (EMR) and present them in a launch window. Selecting a patient in the launch window starts the Sierra Wave application and automatically populates the patient demographics, thereby eliminating double entry of patient data. The HL7 interface module also supports the ability to send results messages back to the EMR at the conclusion of the patient s exam. The results message allows the patient s report document to be imported into the patient s electronic chart. Vendors responding to this sources sought will have to provide the Cadwell Lab s product so you would have to represent yourself as a certified dealer or distributor for Cadwell. An exact product will be required. The statement of Work (SOW) is cited below. All veteran and small businesses replying to this sources sought are to be capable of providing the requirements and service as stated in the statement of work (SOW), you must adhere to the specifications cited on the SOW. Vendors must supply Name of company, address, P.O.C. telephone number and email. Also, submit Duns and Tax ID numbers. Email information to donna.toliver3@va.gov STATEMENT OF WORK CADWELL HL7 INTERFACE LICENSE PURCHASE, DELIVERY, INSTALLATION, AND CONFIGURATION STATEMENT OF WORK PART A GENERAL INFORMATION A.1. INTRODUCTION This contract is for the purchase, delivery, installation, and configuration of HL7 software for an existing Sierra Wave EMG system at VA Butler Healthcare Physical Medicine & Rehabilitation clinic, located on the third floor of the facility at 325 New Castle Road Butler PA 16001; Room # 306-East. All associated installation and training wages shall be included. Contactor will have access to Protected Health Information and a Business Associate Agreement is required using the VHA BAA template. A.2. BACKGROUND Cadwell Laboratories, Inc. (hereafter referred to as the contractor and/or vendor ) is the sole manufacturer, distributor, and service provider of all Sierra EMG systems & the HL7 interface. SCOPE OF WORK This agreement will secure the hardware, software, and installation/configuration services needed to implement the Sierra Wave EMG system. The hardware, software, installation/configuration, and training services covered by this agreement and pricing are covered in the accompanying quote. All components and accessories shall have a minimum of 1-year warranty on parts and labor. STATEMENT OF WORK PART B WORK REQUIREMENTS B.1. Prospective contractor B.1.1. Will provide delivery of all purchased components to VA Butler Healthcare at an anticipated delivery date Thursday, July 6, 2017, that will be coordinated between Prospective contractor, and VA Butler Healthcare. B. 1.2. Will install and configure the following at the direction of the VA Butler PMRS Program manager: B.1.2.1. HL7 Interface License QTY: 1 B.1.3. Will provide sufficient and adequately trained technician(s) to safely install HL7 Interface License, test/configure and perform maintenance on VA Butler s Sierra Wave EMG at current location at 325 New Castle Road, Butler, PA 16001. B. 1.4. Will provide said services within a Monday through Friday time frame, between the hours of 8:00 a.m. to 4:00 p.m. B. 1.5. Will provide preventative maintenance service on the Sierra Wave EMG system while on site to ensure optimal performance of the Sierra Wave EMG. B.1.6. Will provide on-site testing of the device and all installed software to ensure error-free functionality and interconnectivity at the time of installation. B.2. VA Butler Healthcare: B.2.1. Will provide routine problem screening and resolve those problems, as can be locally resolved. B.2.2. Will provide power and as optimal an operating environment as can be reasonably achieved. B..2.3. Will provide network configuration support for the VA Butler side of the VistA-Sierra Wave EMG interface. STATEMENT OF WORK PART C GENERAL REQUIREMENTS C.1. The HL7 sofware shall be installed by personnel certified by the manufacturer to provide the necessary testing and commissioning of the software with the existing EMG device. The EMG system with software shall meet all manufacturer guidelines and specifications prior to final commissioning and acceptance. All accessories will be accounted for & tested on the device. All parts, materials, labor and tools necessary to assemble, install, and test the device and HL7 software shall be included. No equipment or materials shall be stored on site other than during the delivery and installation period. C.2. The contractor shall adhere to the job site requirements listed below: C.2.1. All personnel to adhere to site safety requirements PPE at a minimum to include hard hats, safety glasses, high-visibility clothing, hard soled shoes. C.2.2. All personnel subject to a 30-minute site safety orientation conducted by General Contractor (GC). C.2.3. VA to obtain and maintain current certificates of insurance for each vendor. C.2.4. Vendor responsible for protecting product after installation. C.2.5. Standard work hours are Monday-Friday, 8:00 a.m. to 4:00 p.m. unless otherwise coordinated with the Contracting Officer Representative (COR). C.2.6. Contractor shall provide proof of insurance to COR before any work begins on site. C.2.7. Contractor will be required to comply with physical security guidelines by either checking in with the VA Police each time they come on site to perform contracted services or by obtaining a VA Contractor Identification badge from VA Police. C.2.8. Contractor will be required to get a background investigation, PIV Card, & elevated privileges before performing work. C.3. Training shall be provided for up to 2 personnel to educate them on proper operation and maintenance of the EMG with HL7 software. Training shall be conducted on the site of the installation.
