- Through the Health Care for Homeless Veterans Program (HCHV), The Veterans Affairs Loma Linda Healthcare System (VALLHS) provides a broad range of housing and case management services to Veterans to facilitate their access to medical, mental health and rehabilitative services. HCHV s goal is to engage homeless Veterans in treatment services and place them in a safe and secure environment that supports their rehabilitation goals. Transitional Shelter hereafter referred to as Bridge Housing (BH) Programs are an essential and critical part of the VHA and are vital for providing a gateway to community supportive services for eligible Veterans. The goal of this contract is to provide a secure, appropriately monitored, temporary, community-based placement for those Veterans needing transitional housing within a 30 mile radius of the VALLHS main campus. Transportation is a crucial component of BH Programs to ensure that Veterans are able to check in to the various VALLHS clinics and return to a temporary or permanent placement with minimal barriers. Veterans length of stay within the program shall be determined at the time of intake by the VALLHS staff and should not exceed 90 days. The Contractor shall provide adequate staffing and facility space to ensure delivery of temporary shelter for up to 12 Veterans. The Contractor shall ensure that participants are monitored 24 hours a day, 7 days a week to include all national, religious, and Federal holidays. The Contractor shall ensure that Veterans admitted to the Bridge Housing Program are not housed in other programs located in the same facility. The Contractor shall provide same day placement to Veterans who are eligible for VA healthcare, including Veterans with physical disabilities, active substance abuse, and/or other mental health issues. The Contractor shall provide transportation to and from VALLHS Medical Center, Community Based Outpatient Clinics (CBOC) and for connection to needed community service/resources, Monday through Friday, unless weather conditions prevent safe travel. The Contractor shall provide basic shelter, meals and access to hygiene services and encourage an alcohol and drug free environment. The Contractor shall provide all labor, supervision, supplies, materials, and transportation as required in this contract. The goal of Bridge Housing placement is to temporarily house Veterans while appointments, services, and referrals are in process to secure placement within VALLHS community based transitional residential programs and/or permanent housing.
