- Section L & M SF-30 Amendment of Solicitation AMENDMENT 0001: THE DATE FOR RECEIPT OF OFFERS IS EXTENDED TO MAY 03, 2017, 4:00 P.M. MST. THE OFFER ACCEPTANCE PERIOD (BLOCK 8 OF SF1449) EXPIRES APRIL 28, 2017. THE GOVERNMENT HEREBY EXTENDS THE ACCEPTANCE PERIOD TO MAY 3, 2017 BY 4:00 P.M. MST TO MAKE CHANGES AND INCLUDE FAR CONTRACT PROVISIONS & CLAUSES: CHANGES: -INCLUDED FAR 52.217-5, EVALUATION OF OPTIONS, SEE ATTACHMENT 3. -INCLUDED SECTION L, INSTRUCTIONS, CONDITIONS, AND NOTICES TO OFFERORS OR RESPONDENTS. -INCLUDED SECTION M, EVALUATION FACTORS FOR AWARD. SUBMISSION WILL BE ACCEPTED BY EMAIL AT MARILYN.COWBOY@IHS.GOV AND/OR FACSMILIE AT (520) 295-2626. RESPONSES HAS BEEN EXTENDED TO WEDNESDAY 3, 2017, 4:00 P.M. MST. ONE AWARD WILL RESULT FROM THIS SOLICITATION. ALL OFFERORS MUST SIGN THE SF-1449 AND INCLUDE WITH THEIR PROPOSAL. This is a combined synopsis/solicitation and purpose of this procurement is to provide all personnel, equipment, tools, materials, transportation and supervision (except Government furnished) necessary to perform janitorial services as required by the Government in the solicitation. This acquisition is a set-aside for total Small Business. The North American Industry Classification (NAICS) code for this requirement is 561720 (Janitorial Services) with a size standard of $18.0 million. The solicitation will be awarded as a single award- firm fixed price type contract. The location where the work will be performed, will be at the Tucson Indian Health Service (TAIHS) as specified in the Statement of Work (SOW). The Period of Performance shall for one (1) base year beginning May 26, 2017 through September 30, 2017 with four (4) options year periods. An organized site visit has been schedule for Thursday, April 13 at 10:00 a.m. Participants will meet at: San Xavier Compound, North Parking Lot after Security Guard Shack. This solicitation requires registration with the System for Award Management (SAM) prior to award, pursuant to applicable regulations and guidelines. Registration information can be found at https://www.sam.gov. It is anticipated that the RFP will be issued on April 10, 2017. The anticipated award date is on May 26, 2017. No facsimile or email offers in response to this RFP will be accepted. Potential Offerors hereby notified that furture information about this acquisition, including the solication and subsequent amendments, will be only distributed solely through the Federal Business Opportunities web-site http://www.fedbizopps.gov/.
