- The Department Of Veterans Affairs, Network 15 Contracting Office has a requirement to procure the following Pharmaceutical items listed below for delivery to the Hines, IL CMOP. The request is for Furosemide and Azithromycin to fill for high demand: Description NDC Quantity Furosemide 40 MG 00172-2907-55 2400 Azithromycin 250 MG 00069-3060-30 360 The Product Code is 6505, Drugs and Biologicals; the NAICS code for this solicitation is 325412. All responsible open market sources may submit a quotation, which if timely received, shall be considered by this agency. Submit the RFQ to Karen.Griffin4@va.gov. (Preferred contact method). Solicitation package VA770-17-Q-0301 shall be posted on Fedbizopps on or around 3 May, 2017, and will close no later than 5:00 PM CST on 4 May 2017. ***These dates are subject to change. No solicitation packages will be mailed. Only Bid on items you have in stock, or can supply to the Hines CMOP by ARO May 22, 2017. Vendors must supply a copy of license with quotes. The Drug Supply Chain Security Act applies. Responses must be concise and be specifically directed to the requirement reference above. It is the offertory s responsibility to monitor FedBizOpps for changes or amendments. VENDORS SHOULD BID ON ITEMS THAT THEY HAVE IN STOCK, CAN PROVIDE ITEM QUOTED. Low Bid Award will be available for Immediate Shipment. All pharmaceuticals must have pedigree information sent to the CMOP.
