- Full Statement of Work. There is no section A or B. That will be added when this is incorporated into a purchase order. The Centers for Disease Control and Prevention is seeking a company to produce, film, and edit a short video or series of videos about the prevention of musculoskeletal disorders (MSDs) in Hispanic-owned restaurants. The contractor will furnish all the necessary personnel, facilities, supplies, equipment and travel required to complete the project. The contractor will be responsible for developing a concept, story board, and script for the end product and testing these materials with members of the target audience via focus groups. The contractor will also be responsible for all aspects of video production and post-production. See Statement of Work for complete details including successful evaluation guidance. Remember, your company must be registered at SAM.gov in order to submit a proposal to CDC. The period of performance shall be for fifteen months starting at date of award. If you have questions about anything contained in the Statement of Work, please direct them to the Purchasing Agent at lml3@cdc.gov and they will be forwarded to the project officer/COR for response, which will be uploaded as an attachment to this announcement for use by all bidders. Questions should be submitted NO LATER than May 17, 2017, by noon (12:00 p.m. ET). Your proposal should be submitted by May 31, 2017, by close of business (4:30 p.m. EDT). Your proposal should include two separate documents, your technical proposal and your business proposal, and should be e-mailed to the Purchasing Agent at lml3@cdc.gov. In your technical proposal please address all segments of the Statement of Work and describe any methodology and procedures necessary. There is no special font or page length requirement Your technical proposal need not be lengthy so long as it is complete. In your business proposal you should break out any labor rates for employees working on this project. Travel costs should be broken out separately. This will be a fixed-price award (no time and materials or cost plus fixed fee proposals will be accepted). Award shall be to the company which submits the lowest bid technically acceptable, technical acceptability to be determined by the project officer/COR. At this time, the requirement is reserved for small business only. Large businesses who respond will not be considered. It is your responsibility to monitor FBO for changes to this announcement (posting of questions and answers, extension dates, etc.).
