Amendment- This RFQ has been amended to correct the attached RFQ and Pricelist. The original pricelist included two sub-clins for each year. This is incorrect. Please see updated pricelist in the attached files. No other changes. This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested, and a separate solicitation will not be issued. This solicitation number is VA261-17-Q-0253, is a Request for Quote (RFQ), and is a TOTAL SMALL BUSINESS SET-ASIDE. The associated North American Industry Classification System (NAICS) code is 621330 - Offices of Mental Health Practitioners (except Physicians), and the small business size is $7.5M annually. The Veterans Administration is seeking contractor(s) who are capable of fulfilling the requirement for Re-adjustment Counseling in Ukaih, CA. The Veterans Administration intends to establish a BPA(s) for the attached services with a counselor(s) who are located within the city limits of Ukiah, CA. The counselor must have an established office capable of individual counseling services as well as group counseling services. Please see attached Performance Work Statement for capability requirements. The issuance of a BPA does not guarantee any orders. The requirements in this area vary greatly from week to week. The VA is seeking established counselors with a physical presence in the area who are capable of providing on an as needed / short notice basis. The BPA(s) will be used to order counseling hours as needed to accommodate distressed individuals. Interested parties must be registered in the government data base at www.sam.gov This BPA will NOT have a minimum order. Call values may range from $10.00 to $150,000. The maximum ceiling for this BPA will not exceed $150,000. Payment can be made through credit card or EFT. Most orders will be paid through EFT.
