- SOUTHERN ARIZONA VA HEALTH CARE SYSTEM PURCHASE OF KEYPAD (CYPHER) LOCKS SOILED UTILITY ROOMS PER OIG/CAP FINDINGS STATEMENT OF WORK BACKGROUND An inspection of Soiled Utility/Biohazard Storage Rooms located at the Southern Arizona VA Health Care System (SAVAHCS), conducted in August of 2016 by the OIG/CAP, determined that all of the Soiled Utility/Biohazard Storage Rooms are required to remain locked at all times. Hospital staff required to have access to these rooms must be able to enter with a minimum of effort while maintaining cleanliness and safety. To that end, the Southern Arizona VA Health Care System (SAVAHCS) requires a Vendor to supply Keypad (Cypher) Locks for various Soiled Utility/Biohazard Storage Rooms at the Tucson campus. The SAVAHCS campus is located in at 3601 South 6th Avenue Tucson, Arizona 85723. The SAVAHCS currently utilizes and has expertise in installing and maintaining the Stanley-Best EZ Series Keypad Lock system. The SAVAHCS Locksmith and Carpenters are trained in installing and operating these locks. This contract is for the purchase of Keypad Locks only. VA staff will install the locks. WORK TO BE PERFORMED UNDER THIS CONTRACT Vendor to supply the following locks: Five (5) each, Stanley-Best EZ Series Keypad Lock Model BE/9KZ3-7-DV-KP-STK-626-LC Single keyed lock DV With key 14 Curved with return lever KP Keypad STK Standard strike package 626 Satin chrome finish LC Less core Fifty (50) each, Stanley-Best EZ Series Keypad Lock Model BE/45HZ7-DV-14-KP-626-LC Single keyed lock DV With key 14 Curved with return lever KP Keypad STK Standard strike package 626 Satin chrome finish LC Less core
