- Clauses for solicitation. This is for the procurement and delivery of one (1) 18 foot radome for a satellite tracking antenna system. All inquiries and quotes will be submitted via electronic filing to the following office: david.j.berger@uscg.mil David J. Berger Contract Officer USCG Research & Development Center 1 Chelsea Street New London, CT 06320-5506 Please make sure when you submit your quote that you are registered in the following web site and that your business information is up to date: https://www.sam.gov/portal/SAM/home#1#1 The delivery date for the radomer is NLT 19 May 2017. Proposals shall be received on or before the date and time specified in the solicitation, which is 19 May 2017, 2:00 pm. Offerors shall submit their proposals electronically via email to david.j.berger@uscg.mil. Only electronically submitted proposals will be accepted. Price to include Delivery inspection and acceptance at contractor's facility. In addition the option price for delivery to: Alaska Satellite Facility, 903 Koyukuk Dr. Attention: Samuel R. Nassar, LCDR Fairbanks, Alaska 99775 United States ATTN: MK1 Lauren Goodell (860) 271-2727 samuel.r.nassar@uscg.mil Company's quotes MUST include and must be a part of the following: (1) The solicitation number; (2) The time specified in the solicitation for receipt of offers; (3) The name, address, and telephone number of the offeror and a point of contact; (4) A technical description of the items being offered in sufficient detail to evaluate compliance with the requirements in the solicitation. This may include product literature, or other documents, if necessary; (5) Terms of any express warranty; (6) Price, availability and any discount terms; (7) "Remit to" address, if different than mailing address; (8) A completed copy of the Representations and Certifications at FAR 52.212-3 (Aug 2013), (see FAR 52.212-3(j) for those representations and certifications that the offeror shall complete electronically) (9) Acknowledgment of Solicitation Amendments if any; (10) Past performance information, when included as an evaluation factor, to include recent and relevant contracts for the same or similar items and other references (including contract numbers, points of contact with telephone numbers and other relevant information); (11) If the offer is not submitted on the SF-1449, include a statement specifying the extent of agreement with all terms, conditions, and provisions included in the solicitation. Offers that fail to furnish required representations or information, or reject the terms and conditions of the solicitation may be excluded from consideration; (12) Company Tax Information Number and DUNS Number.
