- Appendix Insulins - Basal Appendix Corticosteroid - HAE Utilization Data Appendix Antiretrovirals - Combinations RFQ HT9402-17-Q-0006 Appendix Antiretrovirals - NRTIs Appendix Antiretrovirals - Other Agents SEC. 743 Pilot Program Appendix Antiretrovirals - INSTIs BACKGROUND: The Department of Defense (DoD) is required by law (10 U.S.C. § 1074g) to establish an effective, efficient, integrated pharmacy benefits program for the Military Health System (MHS) which includes the purchase pharmaceutical agents. The law and implementing regulation (32 CFR Section 199.21) describe the process by which the DoD Pharmacy and Therapeutics (P&T) Committee will consider the relative clinical effectiveness and relative cost effectiveness of pharmaceutical agents in a therapeutic class in recommending the selection of agents for the DoD Uniform Formulary (UF) and, for cost sharing purposes, the classification of a pharmaceutical agent as generic, formulary, Basic Core Formulary (BCF), Extended Core Formulary (ECF), or non-formulary (NF). Prior to the scheduled P&T Committee meeting, a Request for Quotation (RFQ) for pharmaceutical agents within Drug Classes identified for placement on the UF has been issued to obtain quotes from industry. The Defense Health Agency may award Uniform Formulary Blanket Purchase Agreements (UF BPA) and the Uniform Formulary Additional Discount Program Agreements (UF ADP) based on the UF decision. This process is described in the attached RFQ. P&T COMMITTEE MEETING: August 2017. The following drug classes/subclass and agents will be reviewed: Antiretrovirals Sub-Class: Other Agents Aptivus, Crixivan, Edurant, Evotaz, Intelence, Invirase, Kaletra, Lexiva, Norvir, Prezcobix, Prezista, Rescriptor, Selzentry, Tybost, Viracept Antiretrovirals Sub-Class: Combinations Atripla, Complera, Genvoya, Odefsey, Stribild, Triumeq Antiretrovirals Sub-Class: INSTIs Isentress, Tivicay, Vitekta Antiretrovirals Sub-Class: NRTIs Descovy, Emtriva, Trizivir, Truvada, Viread, Ziagen Corticosteroid Sub-Class: HAE Berinert, Cinryze, Firazyr, Kalbitor, Ruconest Insulins Sub-Class: Basal Basaglar Kwikpen U-100, Lantus, Lantus Solostar, Levemir, Levemir Flexpen, Levemir Flextouch, Toujeo Solostar, Tresiba Flextouch U-100, Tresiba Flextouch U-200 RFQ: The RFQ, including UF BPA and UF ADP Appendices are attached herein. The historical utilization data including quantity dispensed and 30 day equivalents by month is also attached herein. PRE-PROPOSAL CONFERENCE: See paragraphs 2.4 and 2.5 of the RFQ.
