- Attachment 3, Sample Price Spreadsheet Attachment 2, Contractor Information Sheet Exhibit E, Equipment List for AFMS Exhibit D, Equipment List for MEDCOM W81K-04-17-R-0003, Solicitation This a Request for Proposal (RFP)/Solicitation to provide non-personal, Medical Device and Medical Device Systems (MD/MDS) maintenance and repair services. The Health Readiness Contracting Office (HRCO) intends to procure MD/MDS maintenance and repair services in support of the US Army Medical Command (MEDCOM) and Air Force Medical Services (AFMS) mission to support healthcare for the Continental United States (CONUS) to include Alaska (AL) and Hawaii (HI). A pre-proposal conference is scheduled for 10 May 2017, 12:00 pm - 5:00 pm, Brooke Army Medical Center, Fort Sam Houston, TX. Interested vendors must register with the contract specialist (below). This is a follow-on requirement to provide MD/MDS maintenance and repair services for MEDCOM and AFMS. This requirement will be solicited as unrestricted, full and open. The Government will issue one solicitation but intends to award two separate Indefinite Delivery Indefinite Quantity (IDIQ) Single Award Task Order Contracts (SATOCs). One SATOC will be to support 32 Medical Treatment Facilities (MTFs) for MEDCOM and the other SATOC will support 62 MTFs for AFMS. Each SATOC will have a five year (60 month) ordering period. Individual task orders will clearly describe MTF-specific MD/MDS requirements and services to be performed. This solicitation will be awarded under FAR Subpart 15.101-1, Tradeoff Process to include applicable evaluation criteria.. In accordance with FAR 22.1003-4(c)(1)(ii); the Service Contract Act is not applicable to the labor of service or repair of medical equipment, however it will apply to the administrative personnel for CLIN X002 in accordance with Service Contract Act (41 U.S.C. 351). The solicitation and subsequent contracts will include contract line items (CLINS) with 5-year ordering periods. The anticipated IDIQ contracts will obligate the government to the minimum guarantee of $1,500.00 for post-award conference. All elements of the RFP shall be submitted by the Offeror with a proposal no later than the date/time shown in block 8 of the Standard Form (SF) 1449, Solicitation/Contract Order for Commercial Items.
