- One-time repair/replacement service Statement of Work THIS IS A NOTICE OF INTENT TO AWARD A SOLE SOURCE CONTRACT AND IS NOT A REQUEST FOR COMPETITIVE QUOTES. Reference #: N0025917N0069. Naval Medical Center San Diego intends to award a firm fixed price sole source contract under the authority of FAR 13.106-1(b)(1)(i), only one responsible source, to: Intuitive Surgical, Inc.; 1266 Kifer Rd; Sunnyvale, CA, 94086-5206. The objective of this sole source award is for a one-time repair/replacement service of two Da Vinci Xi Endoscopes. Salient Characteristics must include: 1. performance by OEM authorized service reps/technicians 2. able to provide OEM-approved replacements in lieu of the damaged scopes as permanent replacements 3. use OEM recommended equipment/tools required to execute all services performed 4. does not negate any manufacturer warranties when servicing and or using manufacturer approved parts in accordance with the attached Statement of Work (SOW) Period of Performance 1 JUN 2017 - 30 JUN 2018 CLIN 0001: 0 Degree 8mm DaVinci Xi Endoscope; Model: 470026-41, Serial: SF1517176, Quantity: 1; Unit of Issue: Each. Includes requirements as stated in the Statement of Work. CLIN 0002: 30 Degree 8mm DaVinci Xi Endoscope; Model: 470027-41, Serial: SF1522123, Quantity: 1; Unit of Issue: Each. Includes requirements as stated in the Statement of Work. Intuitive Surgical, Inc. is the sole distributor of the needed items. The NAICS code for this requirement is 811219; the small business size standard is $20.5 (millions of dollars). This notice of intent is not a request for competitive quotes; however, interested persons may identify their interest and capability to respond to this requirement. All information received by 11:00 AM Pacific Time on May 08, 2017 may be considered by the Government. Responses shall be submitted to Gilberto Esteves at gilberto.a.esteves.civ@mail.mil. The email subject line shall state, "Naval Medical Center San Diego, Intent to Award Sole Source to Intuitive Surgical, Inc."
