- U.S. ARMY CORPS OF ENGINEERS CHICAGO DISTRICT'S SUPPLY AND DELIVERY OF TYPE II PORTLAND "DRY MIX" CEMENT (REPRESENTED AS AN ILLUSTRATED EXAMPLE UNDER NAICS 327999 - DRY MIX CONCRETE MANUFACTURING) This is a combined synopsis/solicitation for commercial items. The Solicitation Number is W912P6-17-R-0004 and is issued as a Request for Proposal (RFP). The NAICS code for this requirement is 327999 with a size standard of 500 EMPLOYEES. COMPETITION FOR THIS ACQUISITION IS FULL AND OPEN COMPETITION AND UNRESTRICTED TO BUSINESS SIZE. AWARD: A firm-fixed-priced Commercial Items Contract will be awarded to the responsive and responsible contractor submitting the lowest aggregate price for the items included in the RFP Bid Schedule. Split or multiple awards will not result from this RFP. Project Description: The project consists of furnishing, delivering and offloading of Type II Portland "Dry Mix" Cement to the Corps of Engineers - Crane Barge Manitowoc, to be used for repair of the Calumet Harbor Breakwater Structure. The Contractor shall be responsible for providing all necessary equipment (i.e. cement dryer/blower system), hoses of an appropriate length and all hose connections to transferring the cement material from the Contractor's bulk material storage container to the Corps of Engineers provided cement silo and/or cement hopper located on a Corps of Engineers provided scow. Place of Contract Performance: Calumet Harbor - The delivery point associated with this Scope of Work is near the Calumet Harbor Boat Shed. This site is located on the north side near the mouth of the Calumet River. The nearest street address of this location is 9169 U.S. 41, Chicago, IL 60627. If the Contractor requires landside access to this point of delivery for any reason, the Contractor must contact the Technical POC in advance to obtain access to the site. REQUIRED SUBMITTALS IN RESPONSE TO THIS SOLICITATION: (1) A signed Proposal (Page 1 of this Solicitation, blocks 30a, 30b, & 30c). (2) A fully completed Section B "Supplies or Services and Prices" (serving as the Offeror's Pricing Schedule), wherein, the offeror shall provide pricing for Item's 0001AA, 0001AB, 0002AA and 0002AB. (3) Fully completed Section K, "Representations and Certifications." (4) A Bid Guarantee in the amount of 20% of the total proposed price or $3M, whichever is less. - Refer to Section L of this document, FAR Clause 52.228-1. (5) Acknowledged Amendments (if Amendments are issued) PROPOSAL SUBMISSION: Regardless of the method of delivery (mailed or hand‐carried), proposal packages shall be enclosed and sealed in an envelope that is clearly marked time. (1) Mailed Proposals: It is the responsibility of each offeror to ensure that its proposal is received by the Chicago District's - USACE Contracting Branch, located at 231 South LaSalle Street, Suite 1500, Chicago, IL 60604 prior to the date and time set for the response. (2) Hand carried Proposals: The offeror may hand carry its proposal. Once reaching the District office located at 231 South LaSalle Street, Chicago, Illinois 60604, the offeror shall CONTACT Mr. Thomas Russell at phone number 312‐846‐5376, or Mike Jones at phone number 312‐846‐5373 to retrieve its PROPOSAL from the Lobby Area of the USACE's office location. 5. RESPONSE DUE DATE AND TIME: Submittals are required no later than 5:00 p.m. CDT on May 10, 2017. Please include the Solicitation No.W912P6-17-R-0004 on all documents submitter.
