- Description
- The Uintah & Ouray (U&O) Service Unit/Indian Health Service (IHS) is seeking qualified vendors who can provide monthly telephone and Internet service for the Uintah and Ouray Service Unit and installation of IP phone system for the Office of Environmental Health & Engineering (OEHE) building. The Contractor shall also provide service and repair as needed. Offers must meet the minimum qualifications and/or requirements set forth within the Request for Quote (RFQ) and must be capable of providing all core services set forth in the Scope of Work (SOW). The Government anticipates an award of a Firm Fixed Price Contract. Hard copies of the solicitation will not be available. Request for Quotes (RFQ), Technical Proposal, and Past Performance References shall be submitted electronically by email (preferred) to the Contracting Specialist at ( Bradley.Platero@ihs.gov ) by the solicitation closing date at 12:00 p.m. (Phoenix Arizona Time) May 19, 2017. Faxed offers will not be accepted. Late proposals will not be accepted. All contractors doing business with the Federal Government shall be registered in the System for Award Management (SAM) database. The website for registration is www.sam.gov. It is the Offerors responsibility to periodically check for any amendments to this solicitation. The North American Industry Classification System Code (NAICS) is 238210 and the Small Business Standard is $15 Million. DUE TO FBO ERROR WE ARE UNABLE TO ATTACHED THE SOLICITATION DOCUMENTS.
