Description
In accordance with FAR 13.106-1 (c )(1), the Department of Veterans Affairs intends to perform oral solicitations for a Visual Field Analyzer for the Portland VAMC. This will be set-aside for SDVOSB s. To be considered eligible for award all SDVOSB s must be registered/verified through Vetbiz.gov. If you wish to be included in this solicitation please respond electronically to laura.davis5@va.gov by 5PM PDT Tuesday, May 9, 2017. This solicitation will be performed Wednesday, May 10, 2017. This will be a brand name or equal to solicitation to the following Items manufactured by Haag-Streit, 1 each: 900 PRO Octopus Perimeter (system includes Octopus Perimeter, Wheelchair accessible table, EyeSuite software, computer, monitor and printer, user manuals and on-site installation.) 1 each: DICOM To be considered technically acceptable, the products must meet the following minimum salient characteristics: Visual field testing equipment with threshold static testing and kinetic testing capabilities. Manual Goldmann perimeter in a modern device with automatic electronic data filing Standard Goldmann sizes, I to V, and intensities, 1a to 4e. Threshold test library including 32, 30-2, 24-2, and 10-2 Semi-automate kinetic perimetry DICOM Conformance and connectivity to Zeiss FORUM PACS currently in use at VAPORHCS eye service. Progression analysis software including Cluster Trend/Polar Trend Software with normative data for statistical analysis of patient data with age matched norms. White on white stimulus testing capabilities. Blue/yellow perimetry SWAP Flicker perimetry for early diagnosis Automated Eye Tracking to improved patient s reliability 7-in-1 printout containing the cumulative defect curve (Bebie curve) or the HFA-style printout. Furthermore, the 4-in-1 printout or the series report Must include a mobile power instrument table that is wheelchair accessible. Equipment must be capable of advanced progression analysis, to include cluster and polar trending. Equipment must be capable of tracking progression of results and of performing end-stage examinations. Equipment operation must improve the overall patient experience. Equipment must be capable of providing sensitive glaucoma analysis. Equipment must operate with the same overall characteristics of a manual Goldmann perimeter unit. Equipment must provide automatic electronic data filing and analysis. Equipment must have ability to ensure proper testing of each visual field point by managing fixation losses due to poor patient interaction or compliance during the examination. Analytical test results must be available quickly to provide optimal patient throughput. Ideally, less than 3 minutes. Equipment must have capability of ensuring repeatable future testing to monitor and track patient eye capabilities, increase of disease symptoms, etc. Vendor shall be an Original Equipment Manufacturer (OEM) authorized dealer, authorized distributor or authorized reseller for the proposed equipment/system such that OEM warranty and service are provided and maintained by the OEM. All software licensing, warranty, and service associated with the equipment/system shall be in accordance with the OEM terms and conditions. This procurement is for new Equipment ONLY; no remanufactured or "gray market" items. All Equipment must be covered by the manufacturer's warranty. *This notice satisfies the requirement at FAR 5.201 to synopsize proposed contract actions.*
