Description
THIS IS A SOURCES SOUGHT ANNOUNCEMENT ONLY. This is Not a solicitation for proposals, proposal abstracts, bids, or quotations. The purpose of this notice is to obtain information regarding the availability and capability of all qualified sources to perform a potential requirement. Responses to the information requested will assist the Government in determining the appropriate acquisition method. The Great Lakes Acquisition Center is planning to solicit for a vendor to provide the following service for the William S. Middleton Veterans Hospital located in Madison Wisconsin: The successful Contractor shall be responsible for all abstracting services to include maintenance of a Tumor Registry within the framework of directives established for approval of Cancer Registries by the American College of Surgeons (ACOS) based on available data. Services shall include but will not be limited to the following: maintaining a repository of records and data related to the treatment; evaluation and study of cancer on a cumulative basis; identifying and abstracting relevant information; and analysis and correction of errors / edits on all patients treated at the VA with reportable neoplastic growths which require prolonged follow-up and observation. The Great Lakes Acquisition Center/ VA Milwaukee is conducting a market survey to determine the existence of potential small business concerns (e.g., HUBZone, 8(a), SDVOSB, VOSB, Woman Owned, etc.) with the capability to accomplish this Service Interested parties shall provide a brief overview of their firm s capabilities and past experience with services similar in scope to this service. All information shall be submitted in Word or PDF format, not to exceed 10 pages, including all attachments. Each submittal shall also include the following business information: DUNS number Organization Name Organization Address Point of Contact (including name, title, addresses, telephone number, fax number and Email address) Business size Socio-economic status ( IF SDVOSB or VOSB provide SDVOSB or VOSB CVE verification) Firms shall identify whether or not they are a small business or other than small business in accordance with the size standard for NAICS code 518210. If another NAICS code may be applicable, please identify the NAICS in your capability statement. All information must be received no later than: May 12, 2017, 2:00 PM, CST. The Government reserves the right to use any non-proprietary technical information in any resultant solicitation(s).
