THIS IS A SOURCES SOUGHT NOTICE. (A Sources Sought is defined as not an actual bid or proposal solicitation; instead, it's a solicitation of interest. Think of a Sources Sought as market research being conducted by a government agency to determine what the capabilities and interests of the marketplace are).This is not a request for actual bid or proposal solicitation. A solicitation will not be issued at this time. The Department of Veterans Affairs, Phoenix VA Health Care System (PHOVAHCS) has a requirement for 647-002 Midmark 647 Barrier-Free Power Podiatry Procedures Chair, programmable w/receptacles also for 002-1121-241 Upholstery Top - ultra premium, Ultra Pacific Blue. The intent of the procurement is to award a firm-fixed price contract. Phoenix VA Health Care System (PHOVAHCS): 650 E. Indian School Road Phoenix, AZ 85012 The applicable NAICS code is 339112 Surgical and Medical Instrument Manufacturing. Small Business manufacturers for this NAICS code have a size standard of 1000 employees.   The resulting contract will be a firm fixed price order. The term of the contract is intended to be 30 calendar days ARO, with no options included. It is anticipated that a Request for Quotes will be issued in May 11, 2017. Award of a firm fixed price contract is contemplated by May 20, 2017. This notice is published to conduct market research to determine if there is a sufficient number of certified Service Disabled Veteran Owned Small Businesses (SDVOSB), Veteran Owned Small Businesses (VOSB), Small Businesses (SB), or Large Business capable of performing the requirements to warrant a socio-economic set-aside or full & open competition. It is strongly recommended that interested parties register in the US Department of Veterans Affairs Vet Biz database and the Small Business Administration (SBA) database. Registration may be accomplished at http://www.vip.vetbiz.gov/ and www.sba.gov/, respectively. Interested parties responding to this sources sought request shall submit the following information at a minimum: (1) Company name and address, point of contact with phone number, and DUNS number; (2) Documentation relating to capability of performance; (3) Intentions of subcontracting program to include set-asides if applicable; (4) Proof of registration in the VA Vet Biz or SBA websites or intention of registration prior to submission of offers to a solicitation and (5) Any other pertinent company documentation. The response date to this Sources Sought notice is May 11, 2017 at 10:00 pm EST. This market research is for informational and planning purposes only to determine if a socio-economic set-aside, or full & open competition, or sole-source procurement is appropriate. ALL FIRMS ARE INSTRUCTED NOT TO CALL THE INDIVIDUAL VA MEDICAL CENTERS REQUESTING MORE INFORMATION. Statement of Work 3 Midmark 647 Barrier-Free ® Power Podiatry Procedures Chairs 1.0 Background: With the addition of new Podiatrists to work at the CBOCS, these chairs are needed. We are requesting one chair each for the South East, Thunderbird, and North West CBOCs. This equipment is a standard Podiatry Chair that is routinely used in podiatry practices and in our main hospital Podiatry Clinic. With Podiatrists now operating in several of our CBOCs, we need to provide them with this specialized equipment for the care of their patients. Podiatry Chairs are specialized in that they are hydraulic chairs that fully recline (required for many clinic procedures) and can achieve Trendelenberg (inclined) position in which the feet can be elevated above that of the head and or hips to allow the clinician to visually inspect the bottom of the feet. This is a critical function in our clinics. Achieving such position is also important in cases of vasovagal responses (fainting) or during severe hypoglycemic episodes wherein patients sustain prolonged or abbreviated states of hypotension. Rapid repositioning of such patients is critical to their resuscitation. This Podiatry Chair also allows easy access for our frequently disabled or impaired veteran patients who have difficulty standing or transferring from wheelchair to chair. In this regard, no step- up is required to achieve a stable sitting position on the chair. The period of performance will be 30 days after receipt of award. 2.0 TASKS: The vendor, will deliver all necessary equipment to the below shipping address. Vendor will offer instructions on maintenance, cleansing, and customer service support regarding any issues or concerns. The importance of this equipment is to expand and improve the quality of services provided to veterans. The life expectancy of this equipment, barring accidental misuse or abuse, would sustain Surgical Services providers for several years. 3.0 Delivery and Shipping Information: The contractor shall provide the following for shipment and delivery of equipment: All items shall be shipped to the following location: Surgical Services PVAHCS 650 E. Indian School Road, Mail stop 112 Phoenix, AZ 85012-1892 4.0 TRAVEL: The contractor is responsible for all travel-related costs associated with any onsite delivery of the Doppler's.
