- Statement of Work Background: The VA Connecticut Veterans Health System maintains a Heidelberg Engineering, Inc. HRA Plus OCT Spectralisis eye diagnostic system in VACT. The present unit has a defective angle fixation lamp and requires a complete factory rebuild and calibration. Objective: This contract is to replace the fixation angle light in the present unit EE 85474. In order to accomplish this part replacement it will be necessary to receive a loaner unit, have this unit removed and transported to the Heidelberg factory in Germany. Service has indicated that the only proper process to replace the needed part is to perform a full factory rebuild. This will allow for the highly critical calibrations necessary to align the lasers properly.Due to the fact this is a highly sensitive and critical opthamology technology. Scope of Work: The services required include: Removal of the OCT Spectralis unit EE 85474 and replacing temporarily with a loaner unit. Includes the cost of the loaner, removal of the primary unit and the time and travel to calibrate the loaner. Cost of service call time and travel to diagnose the original error Shipment and transportation of the OCT unit to the factory in Germany Replacement of critical part, fixation angle light and all other required parts and materials necessary to return the unit to OEM specifications Return shipment/transportation of OCT unit EE 85474 unit to USA Return delivery of unit to VACT, installation and full calibration of unit insuring it meets all OEM specifications Removal of loaner unit 90 Day warranty on all parts and labor Final inspection and verification of proper operation by clinical vision center staff with full service ticket completed and signed by CE and clinical staff 3. Quality of Service Safety Regulations: All applicable safety regulations shall be observed during all work of this contract. The VA Safety Officer shall have full authority to see that the contractor obeys all safety rules and regulations relative to the fulfillment of this contract. The VA Radiation Safety Officer (RSO) shall have full authority to see that contractor fully conforms to all TJC, NRC agency requirements and any other jurisdiction deemed applicable. Scheduling and Hours of Work: All work in areas involved shall be scheduled with the Biomedical Equipment Support Specialist, John Klemenz, in Clinical Engineering (or designee) and the staff in the Lab area.
