- Attachment 2 Past Performance Information Collection Sheet. Attachment 1 Individual(s) Authorized to Bind the Company Solicitation for Compounded Pharmaceuticals Joint Base Lewis-McChord Health Contracting Cell (JBLM-HCC) is soliciting for Compounded Pharmaceutical Supplies to Support the Department of Pharmacy at Madigan Army Medical Center (MAMC), Tacoma, WA. The types and quantities of compounded pharmaceuticals required are contained within the solicitation. A set number of cases of each of the required compounded pharmaceuticals will be expected for delivery monthly. The Contractor is responsible for ensuring timely delivery of the supplies. The Contractor will be expected to replace all damaged and unusable supplies at no cost to the Government. This requirement will be for 12 months. Note: All questions regarding this requirement shall be submitted via email.
