Description
This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. The solicitation number is VA262-17-Q-0916 is issued as a request for quote. The North American Industry Classification (NAICS) Code for this acquisition is 333318, with a business size standard of 1000. Please identify your business size in your response based upon this standard. This requirement is put forth as Unrestricted. The VA Long Beach Healthcare Center seeks to issue a purchases order for the Bio-Medical Department. The Contractor shall furnish all materials/services at the Department of Veterans Affairs, Long Beach Healthcare System, 5901 E 7th Street, Bldg. 126, 3rd Floor, Long Beach, CA 90822. Request item listed below: The Department of Veterans Affairs (VA), VISN 22 Network Contracting Office, is seeking sources that can provide services for the following items listed below that at a minimum meets the following salient characteristics with a brand name product only for the VA Long Beach Healthcare System. ITEM NO. DESCRIPTION QUANTITY UNIT ESTIMATED OR STOCK NO. UNIT COST ----------------------------------------------------------------------------- 1 CRI DUAL DIST PUMP WITH AUTOMATIC CONTROLLER 3HP, 460V, 3PH 1 EA 2 LABOR HOURS (AFTER HOURS) 7 HR 3 MISC. PLUMBING AND ELECTRICAL PARTS 1 EA The information identified above is intended to be descriptive, not restrictive and to indicate the quality of the supplies/services that will be satisfactory. It is the responsibility of the interested source to demonstrate to the government that the interested parties can provide the supplies/services that fulfill the required specifications. STATEMENT OF WORK 600-17-3-668-0418 WATER PURIFICATION SYSTEM BOOSTER PUMPS REPLACEMENT DEPARTMENT OF VETERANS AFFAIRS LONG BEACH HEALTHCARE SYSTEM ENGINEERING/BIOMED 1. GENERAL OBJECTIVES AND REQUIREMENTS 1.1 General: This service is intended to perform corrective maintenance (replace the booster pumps) on the water purification system located at the Department of Veterans Affairs Long Beach Healthcare System, 5901 E. 7th Street Long Beach, CA, 90822. 1.2 Description of Services/Scope of Work: Contractor shall provide all necessary labor, transportation, equipment, materials, personal protective equipment, supervision, other items and non-personal services required to replace the booster pumps on the hemodialysis water purification system described in the Statement of Work for the Department of Veterans Affairs Long Beach Healthcare System. 1.3 Physical Security: The Contractor shall be responsible for safeguarding all government equipment, information and property provided to the Contractor personnel while performing the services described herein. 1.4 Period of Performance: The period of performance shall be for a (30) thirty-day period from the contract award date. The replacement of the pumps shall be done outside of the hemodialysis normal working hours, outlined below, to avoid any interruption to patient care. 1.5 Type of Contract: The government will award a Firm-Fixed Price (FFP) contract. 1.6 Designated Point of Contact: The Designated Point of Contact will provide unlimited communication with the Contractor in response to the hemodialysis water purification system. The Designated Point of Contact shall not provide any formal direction and/or guidance to the Contractor on the performance of the booster pumps replacement on hemodialysis water purification system. The Designated Point of Contact will be on the premises where the services will be performed. 1.7 Contract Administration: The Contracting Officer is the only person authorized to approve changes or modify any of the requirements under this contract. The Contractor shall communicate with the Contracting Officer on all matters pertaining to contract administration. Only the Contracting Officer is authorized to make commitments and/or issue changes that will affect price, quantity or quality of performance of the contract. In the event the Contractor effects any such change at the direction of any person other than the Contracting Officer, the change shall be considered to have been made without authority and no adjustment will be made in the contract price to cover any increase in costs incurred as a result thereof. All changes to the contract will be issued via an amendment and/or modifications in writing from the Contracting Officer to the Contractor. 1.8 Invoices: Invoices are to be submitted electronically; in arrears of the start date of the services that were performed. The Department of Veterans Affairs utilizes a third party invoicing service provider (Tungsten Network) for handling electronic invoices. All invoices should be electronically submitted to the Tungsten Network website: http://www.tungsten-network.com/us/en/. Electronic invoice can be submitted at no additional cost to the Department of Veterans Affairs Vendors. If NOT registered with the Tungsten Network (formerly OB 10), please call (1-877-489-6135) for registration and/or assistance. 1.9 Work Stoppage/Delays: The Contracting Officer shall be notified at the earliest time available, but, no later than the next business day, via phone of the Contractor s knowledge of any conditions which may adversely affect the Contractor s ability to perform under the terms and conditions as stated in the specifications of this requirement. For example, vehicular problems, licenses, or anything that might jeopardize the terms and conditions of the contract should be reported to the Contracting Officer immediately. 2. TASK SPECIFICATIONS 2.1 Description of Work: The Contractor shall provide qualified inspectors, labor, technicians, equipment, tools, materials, transportation, supervision, and any other items, and non-personal services deemed necessary to replace the replace the booster pumps on the hemodialysis water purification system. The contractor shall perform all necessary testing (i.e. disinfection) upon the completion of the pump replacement to ensure the hemodialysis water purification system is safe for use. Upon completion of the job, the report will be generated and submitted to the Engineering/Biomed Supervisor. Service Location: Department of Veterans Affairs Long Beach Healthcare System 5901 E 7th Street Long Beach, CA 90822 2.2 Service Requirements: The Contractor shall be responsible to provide a new booster pumps (CRI Dual Dist. Pump with automatic controller 3HP, 460V, 3PH) and all necessary plumbing and electrical parts required to replace the booster pumps on the hemodialysis water purification system. Upon completion of the job, all work performed and testing results must be documented in a fashion acceptable manner to the Department of Veterans Affairs. 2.3 General Services Information: 2.3.1 The Department of Veterans Affairs Hemodialysis Department Hours of Operations: Normal hours: Monday Saturday, 05:00 a. m. to 8:30 p. m. After Hours: If needed, this must be coordinated with the Designated Point of Contact and notify the Contracting Officer. 2.3.2 All equipment shall be restored to full service; immediately following completion of the job. 3. ACCESS BADGING AND PARKING The Designated Point of Contact is responsible for obtaining access badges and securing parking for Contractors, in accordance with the Department of Veterans Affairs Long Beach Healthcare System Police Department Parking Policy and Personnel Suitability and Security Program. 4. DELIVERABLES - Contractor shall provide a complete report and/or documented forms of all work performed. 5. GOVERNMENT FURNISHED EQUIPMENT AND SERVICES - The Contractor may use the government utilizes (electricity, water) necessary for the performance of services described herein. The Contractor will not have access to the Department of Veterans Affairs Long beach Healthcare System information system or access to government computers to access online information while in performance of the services described in the Statement of Work.
