- Please see Questions and Answers which werereceived during solicitation period: Please provide clarification to the following questions: Q: What is the dollar magnitude of this solicitation? A: It depends on Medical Provider of this service and locations availability. Q: Sect 1.2 Primary location (Phoenix) is the DOE providing the office space? A: No Primary locations. Phoenix location has more employees who will attend Medical Annual exams. However, offeror must list 4 locations on a bid schedule. Q: Is office space available for rent? If so can you provide sq footage, rooms details, etc. Is there any utilities cost? Will we have to include furniture aside from medical equipment? A: No space availability. It has to be provided by Medical provider, including all needed equipment Q: Sect 1.6 Please provide clarification on the type of bond that is being required to obtain. A: Every Medical Provider has different type of bonds. However, it has to be submitted along with proposal. Per Solicitation WES-H-1023: WES-H-1023 REQUIRED INSURANCE (WAPA, FEB 2008) a. The contractor shall procure and maintain during the entire period of his performance under this contract the following minimum insurance: (1) Workman's Compensation Insurance in accordance with the laws of the state where contract performance occurs; Occupational Disease Coverage; and Employees Liability Coverage. (2) Comprehensive General and Automobile Liability Insurance with the following minimum coverages: (i) GENERAL LIABILITY (A) Bodily Injury $500,000.00 per occurrence. (B) Property Damage $500,000.00 each accident; $1,000,000.00 in the aggregate. (ii) AUTOMOBILE INSURANCE (A) Bodily Injury, $200,000.00 per person; $500,000.00 per occurrence. (B) Property Damage, per occurrence: $20,000.00. Q: What is the technical criteria? A: This information is stated on page 36 and Position Description that is identified in the WAPA O. 340. : WES-M-1009 Quotation Evaluation - Contract Award (WAPA, FEB 2000) a. The Government will evaluate all quotations received in response to this solicitation, and award a contract to the responsible offeror whose quote, conforming to the solicitation, will be the most advantageous to the Government, considering price and other factors as specified herein. FACTORS: (1) PRICE per 4 different locations, and (2) ABILITY TOMEET TECHNICAL REQUIREMENTS PER PWS. Q: On the Bid sheet it states Henderson AZ did you mean Nevada? A: Yes, Henderson, Nevada as well as the other locations identified. AZ was a typo. Q: Also what WD would you use for Nevada? A: It will be WD 15-5593 (Rev.-3) ******************************************************************** Amentment 003: New due date to submit proposals is 05/15/2017. (ST) ****************************************************************** Amendmnet 001 03/21/2016: Due to up-loading issues with the server, this amendment is to re-load all documents again to FBO.GOV ********************************************************************* This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Federal Acquisition Regulation (FAR) Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued separate from this announcement. Solicitation number DE-SOL-0011030 is issued as a Request for Quotation. Solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-95. A single contractor will be awarded using Simplified Acquisition Procedures. This is a competitive 100% Small Business set-aside. The acquisition is subject to the North American Industry Classification System (NAICS) code of 621399. The U.S. Department of Energy, Western Area Power Administration, Desert Southwest Region, requires Medical Annual Exam for our linemen, craftsmen, electricians, etc. The number of staff to have annual mandatory medical evaluations is no more than 150 people. PWS (Performance Work Schedule) is attached. Offeror must be registered in the SAM database. If the offeror is not registered in this database, it may do so through the SAM website at: https://www.sam.gov/portal/public/SAM/. All quotes must be valid for 60 days. Only quotes containing all items listed in the solicitation will be considered for award. No partial quotes will be accepted. If a change occurs in this requirement, only those offerors that respond to this announcement within the required time frame will be provided any changes/amendments and considered for future discussions and/or award. Written offers and the above required information are due no later than Friday, April 28, 2017 14:30 PM Pacific Time to Department of Energy, Western Area Power Administration, Desert Southwest Region, P.O.
