- PERFORMANCE WORK STATEMENT 1. The Contractor shall perform state-of-the-art, full-support HD services to beneficiaries of SLVHCS utilizing Contractor s staff and equipment. Services will include acute HD for critically ill patients. The Contractor shall provide the necessary qualified clinical and technical staff to perform and supervise all inpatient HD treatments. The health care provided shall be delivered in a manner consistent with the needs of the patients and standards of quality provided to the Contractor s non-VA patients. The Contractor shall have a minimum of three years experience providing contracted HD services at a VA or academic hospital. The Contractor shall adhere to all federal and state laws and regulations in effect during the term of this contract. Contractor Furnished items (to be used for Government contract work only): New, or newly refurbished within the previous 12 months, state-of-the-art HD machines; All necessary non-physician HD staff; All necessary HD equipment and supplies; and All items identified in paragraph 13: INPATIENT HD TREATMENT PRICE PER TREATMENT. VA Furnished Items: Space for providing HD services; VAMC-SLVHCS access badges; Patient chairs in the Acute HD Unit; Basic facility amenities (e.g., electricity, restrooms, etc.); Housekeeping/cleaning services; Computers and Internet access; Drugs, as required/prescribed per VA physician order, including but not limited to: Apresoline (hydralazine) Formulary Antiarrythmics Formulary Insulins Formulary Local Anesthetics Mannitol Dopamine Formulary Pressor drugs Lidocaine Diphenhydramine Solu-cortef (Hydrocortisone) Lanoxin (Digoxin) Formulary Antihypertensives Glucose Tablets Dextrose Propranolol Verapamil Levophed Non-Intravenous Antibiotics (when used at home by a patient to treat an infection of the catheter site) Normal Saline Heparin Protamine Epogen Calcitriol Activase And any drugs used as a substitute for medications above, prescribed at discretion of VA physicians. 2. The Contractor shall utilize Registered Nurses to perform all inpatient treatments. Each nurse performing those services shall be responsible for the composition of the dialysate, IV solutions, blood and blood products and medications as ordered by physician into the dialyzer blood line system necessary for treatments. The Contractor shall provide compatible water required for dialysis treatments and shall conduct all water testing and equipment maintenance. Water purity testing must meet the standards of the most current guidelines cited by the Association for the Advancement of Medical Instrumentation (AAMI). The Contractor shall be responsible for patient monitoring during the dialysis procedure. 3. Dialysis access shall be placed by SLVHCS. Dialysis consent will be completed by SLCHVS before HD treatment. 4. For emergency dialysis (to be available 24 hours per day, seven days per week), the Contractor shall respond with appropriate personnel (be on site at SLVHCS) within one hour from placement of dialysis access (placement of dialysis catheter) and the call from the ordering SLVHCS physician. 5. The SLVHCS nephrologist shall manage the patient s primary medical and/or renal problems. 6. All emergencies (Code Blue, cardiac arrhythmia, respiratory failure, etc.) will be attended by SLVHCS staff. 7. Contractor shall provide new, or newly refurbished within the previous 12 months, state-of-the- art HD equipment and shall maintain and document all required preventive maintenance and repairs; shall comply with all relevant regulatory agencies and standards for its equipment; shall be responsible for monthly bacterial water testing and yearly chemical water testing, and shall assure compliance with AAMI guidelines; shall be responsible for quality assurance requirements related to water testing and other elements. The Contractor shall be responsible for all maintenance, upgrades/updates and servicing of Contractor-owned equipment during the contract period. 8. The Contractor shall comply with all applicable requirements of Federal, State, and local hazardous waste regulations while performing under this contract. 9. The Contractor shall document the services provided via a paper HD flow sheet, shall maintain patient information, medical records including results of tests, and shall follow appropriate procedures to ensure that patient confidentiality rights are not violated in accordance with applicable State and Federal confidentiality laws. In the event the nephrologist issues a verbal or telephone order to the registered nurse, that order shall be written down and read back to the provider to ensure accuracy. The order shall be signed by the provider no more than 72 hours after it is given. 10. The Contractor shall document the HD procedures on each patient treatment (duration of dialysis, type of dialysate used, and any difficulty with the procedure) on the HD flow sheet. Any ancillary reports will be completed as necessary. This will be in addition to notes of any notable change in a patient s condition. 11. The use of disposable, single-use only items for providing service under this contract is mandatory. 12. Contractor shall assure compliance with all relevant regulatory agencies and standards for their personnel, including but not limited to Joint Commission (JC) or equivalent accreditation program, the Occupational Safety and Health Administration, and the state of Louisiana, for all HD services rendered under this contract. If the Contractor is not JC-accredited, Contractor shall provide documentation, upon request, that ensures it has the resources to meet the quality requirements of the contract. Such documentation must be approved by the Chief, Quality Management, SLVHCS. This documentation shall be provided with bid proposals. 13. INPATIENT HD TREATMENT PRICE PER TREATMENT: a. All HD treatments will be billed and paid in accordance with the SERVICES DESCRIPTION price per treatment. The price per treatment shall include all services, equipment, supplies, etc., that are necessary to complete HD treatments. As part of the HD treatment, the Contractor is responsible for monitoring access. If an access malfunction occurs, the SLVHCS nephrologist shall be contacted for orders to provide appropriate patient care. The SLVHCS nephrologist shall be available 24/7/365. Contractor staff time used to perform HD services, including time to administer blood, is included in the price per treatment. b. Routine items and services related to the HD treatment included in the contract price per treatment: Medically necessary dialysis equipment and dialysis support equipment; Purchase and delivery of all necessary dialysis supplies; Flushing of dialysis access and any supplies used; Staff time used to administer non-routine parenteral items; All dialysates, artificial dialyzers, bloodlines, and other supplies; Dialysate additives (e.g. Bicarbonate); Cardiac monitoring; and Dressing changes of dialysis catheter or peripheral dialysis access. c. Medications prescribed as part of HD treatment shall be administered by the Contractor RN. This administration (Contractor staff time) is covered in the price per treatment and may not be billed separately. 14. The Contractor is required to provide documentation on the HD flow sheet for all treatment/services provided on the day of the service. 15. UNCOMPLETED TREATMENTS: If an HD treatment is started, i.e., a patient is connected to the machine and a dialyzer and blood lines are used, but the treatment is not completed for a valid reason, the Contractor will be paid based on the full price per treatment. Such situations must be fully documented. 16. WORK HOURS: a. The services covered by this contract will be furnished by the Contractor as defined herein. The Contractor may be required to furnish such services during off-duty hours or national holidays, under CLINs: 0001c, 1001c, 2001c, 3001c and 4001c as described below. b. The following terms have the following meanings: Work hours: Monday through Saturday, 8:00 a.m. 4:30 p.m. National Holidays: The ten observed Federal holidays are: New Year s Day; Martin Luther King s Birthday; Presidents Day; Memorial Day; Independence Day; Labor Day; Columbus Day; Veterans Day; Thanksgiving; Christmas; and Any other day specifically declared by the President of the United States to be a national holiday. Off-duty hours: Monday through Saturday, 4:30 p.m. 8:00 a.m.; all day/night Sunday and holidays.
