- Page 1 of 5 This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. This solicitation VA251-17-Q-0588 is issued as a request for quote (RFQ). The North American Industry Classification System (NAICS) Code 337127 Institutional Furniture Manufacturing and a Business Size Standard of 500 employees is applicable. This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular (FAC) 2005-89. No telephone requests will be accepted. This is a brand name or equal request. Quote is requested for the following: ITEM INFORMATION ITEM NUMBER DESCRIPTION OF SUPPLIES/SERVICES QUANTITY UNIT UNIT PRICE AMOUNT 0001 Sleep recliners per below SOW 24.00 EA $_____________ $______________ GRAND TOTAL $_____________ STATEMENT OF WORK PURPOSE. This statement of work describes the requirements for the procurement and delivery of furniture and furnishings, for the VA Northern Indiana Health Care System (VANIHCS), Marion Campus, Building 185 Mental Health Patient Dayroom & Quiet Rooms. BACKGROUND. Nursing Services in conjunction with the Interior Design team are requesting replacement furnishings for the three Mental Health Patient Dayrooms & Quiet Rooms in Building 185. These are common areas where Veterans spend majority of their time in the locked units reading, lounging, watching TV and more as they receive care at the Marion facility. The existing furniture in the units is in poor condition. The upholstery on most of the furnishings is torn beyond repair and the locking mechanisms on recliners within the units no longer work effectively. The current condition of the furnishing presents both a patient and staff safety issue, making it inadequate for Veteran use as well as difficult for EMS housekeeping staff to clean presenting an infection control issue. SCOPE OF WORK. This project shall consist primarily of delivery of all furniture and furnishings outlined in this document. The Contractor shall provide the following products and related services. Manufacturing and delivery of products. The Contractor shall be responsible for any damage to the furniture that occurs due to Vendor/freight error or neglect. The Contractor will be responsible for the coordination for delivery to the VANIHCS. The Contractor shall submit an estimated schedule of delivery. This is a brand-name or equal procurement. All products shall be brand name or equal to the products listed in this document. Products shall comply with the salient characteristics and the requirements as defined in Section 4. If a product listed in the SOW has been discontinued or is no longer available, the Contractor shall provide a currently available product that is equal in terms of essential salient characteristics to the product in the SOW. In accordance with FAR 52.211-6, the Government has elected to use brand names and specific products in an effort to assist the Contractor in the bidding of this project. The brand names and products specified are being used to establish a minimum quality level for various items and products. The Government does not intend or imply that by providing vendors or manufacturers names in the specification that subject products or items are the only items that will be acceptable to the Government. All products and items, whether named or not named, must clearly meet the salient characteristics of the specifications identified. PRODUCT. This project consists of the procurement, delivery, set up and installation of: Wieland Accord Sleep Recliner, Wide, Model 65W981S or equal Qty: (24) General Requirements. All furniture must meet the following salient characteristics as specified below. Colors and Fabrics. All fabric and finish color selections shall be selected and approved by the VANIHCS Interior Designer. All products shall be designed with durability to meet the demands of the healthcare environment, including abrasion resistance and anti-microbial finish to support the reduction of fungal growth, spores from germs, microbes and bacteria, and provide tight cushion or clean out space to assist with the cleaning of products, and prevention of buildup of debris and germs. All finishes shall be engineered for heavy use and frequent cleaning using hospital-grade cleaning substances. Seating Fabrics: All seating fabrics shall comply with the following minimum standards. 100% PVC free Bleach Cleanable Anti-Microbial ASTM D4167 Wyzenbeck Method (2002), minimum 500,000 double rubs ASTM D3511 Class 4, Brush Pill Test Colorfastness to Light, AATCC 16 option 1 or 3 (2003), Grade 4 minimum at 40 hours California Technical Bulletin 117 All products within each room or space shall have compatible finishes and colors to provide an aesthetically pleasing solution. Dimensions. All dimensions defined in the SOW are nominal dimensions. The actual dimensions of each product or component may vary slightly from the nominal dimensions listed, as long as the products provide the same functionality as products of the exact dimensions, and as long as the products fit within each room or space. Warranty. All products shall be designed for healthcare application and covered with manufacturer s standard warranty for healthcare use 3 shift, 24-7/365 days, with minimum 10-year warranty, including parts and labor. Specific warranty requirements shall be as listed below. All products shall meet indoor air quality and low emitting VOC requirements in accordance with the GREENGUARD Indoor Air Quality Certification program, or equivalent or better environmental standard. The Contractor shall provide documentation showing that their products are GREENGUARD certified or are certified in accordance with the equivalent or better standard. All products below shall be quality and style of Wieland or equal product: Wieland Accord Sleep Recliner, Wide, Model 65W981S or Equal: Recliner Size (Overall): 30.75 W x 34.25 D x 46.25 H. Seat: 23.5 W X 21 D x 20 H. Recliners shall have thermoform arm caps, a recessed recline assist lever, auto-align and auto-release casters and a separate braking mechanism that completely retracts when not in use and grips the floor securely and provides additional stability when in use. All components shall be assembled with tamper resistant hardware Seating shall have fully renewable and upholstered seat and back covers, with standard moisture barrier. Weight capacity and strength performance: 500 lbs. ***Manufacturer is Wieland or Equal- FAR 52.211-6 Brand Name or Equal*** FOR ALL PRODUCTS: Must meet salient characteristics outlined above when quoting an or equal item FAR 52.211-6 Brand Name or Equal. Or equal items quoted should be similar to the requested dimensions, color scheme, function, and quality. Or equal products must include manufacturer s product brochure including specification information and all proposed finish selections. Actual finish samples if quoting an or equal item must be submitted for review by VANIHCS Interior Designer for technical and aesthetic review. FAILURE TO CLEARLY NOTE THAT THE PRODUCTS BEING QUOTED ARE THE REQUESTED BRAND NAME, OR FAILURE TO SUPPLY OR EQUAL PRODUCTS SPECIFICATIONS AND INFORMATIONAL LITERATURE SHALL CONSTITUTE A NON-RESPONSIVE BID AND WILL NOT BE FURTHER EVALUATED FOR AWARD. DELIVERY. Drop ship to Marion VA warehouse facility. VANIHCS shall only receive fully assembled seating from manufacturer. Delivery is requested by August 4, 2017. SUBMISSION OF QUOTATION. Quote Submission: The contractors shall submit complete emailed electronic copy portable document format (.pdf) or Microsoft Excel (.xls) format of: Volume 1: (technical) drawings, (floorplans, isometrics, shop drawings, typicals etc.) of proposed furniture items with dimensions per bill of materials with product subtotal complete manufacturer s product specifications product literature finish samples sustainability certification documents complete testing results / certificates warranty self-certifying statement confirming ability to meet project deadlines. Volume 2: (pricing) price quote Submissions can be sent in multiple emails to avoid computer system email size limitation. Quotes submitted by any other method will not be considered. All proprietary information shall be clearly marked. The use of hyperlinks in quotes is prohibited. Late quotes will not be accepted for evaluation. The contractor is responsible for confirming the Government s receipt of the contractor s quote. EVALUATION CRITERIA. Order will be evaluated on: The separate, typed document from the VA Solicitation that matches the Solicitation document exactly per each line item. Products or services that include multiple parts shall be listed within each line item. No additional line items will be added. PDFs of detailed and dimensioned: Plan, 2-D and 3-D drawings of each product with multiple parts list to represent exactly what is being included in order and to determine accuracy. Cut sheets with product detail for each line item. Labor/Services appropriate to the scope of work. Project staffing plan appropriate to scope of work. Sustainability Certification Price Business size- preference will be given to SDVOSBs and VOSBs. Invoices shall be submitted in accordance with Federal Regulations 52.212-4(g) to VA Financial Services Center through the Tungsten (OB10) electronic invoicing system. The provisions at FAR 52.212-1, Instructions to Offerors-Commercial Items, applies to this acquisition. Offerors shall submit with their offer, a completed copy of the provisions at FAR 52.212-3, Offeror Representations and Certifications-Commercial Items or confirm this has already been completed in their SAM registration. The clause at 52.212.4 Contract Terms and Conditions-Commercial Items, applies to this acquisition. The clause at 52.212-5, Contract Terms and Conditions Required to Implement Statues or Executive Orders-Commercial Items, applies to this acquisition which shall include the following clauses: 52.222-21, 52.222-26, 52.222-35, 52.222-36, 52.222-37, 52.232-34. Offerors shall submit their quotation by e-mail to Brooke.Hansen@va.gov. Offers are due no later than 4:00 pm. EST, 17 May 2017. NO LATE QUOTES WILL BE ACCEPTED OR CONSIDERED.
