February 9, 2017 5 of 10 Statement of Work - Electromyography (EMG) February 1, 2017 1 of 10 This is a REQUEST FOR INFORMATION only. Market research is being conducted to determine the amount of qualified and capable businesses that can provide the items requested below. If you believe your firm is able to provide the items requested below, you are encouraged to respond by the due date with all of the required information indicated below in order to be considered. NACIS: 334510 Size Standard: 1250 employees SALIENT CHARACTERISTICS-ELECTROMYOGRAPHY (EMG) Brand Name or Equal Objective: The overall system shall be one, vendor-provided, solution comprised of four (4) new EMG devices; plus the required software, acquisition/review workstations, server(s), accessories, and interface to VISTA/CPRS as specified and enclosed on this Statement of Work (SOW) document.   Scope of Procurement: This project will include the EMG units, carts, accessories, computer/server hardware, software, and VISTA/CPRS interface as specified in this document. In addition, the project includes other services including deliver, installation, set-up, clinical training, technical training, education and technical documents, and trade-in/disposal of old equipment. Period of Performance Overall period for performance for the project shall be negotiated with the vendor. Work shall be performed during normal business days at the Minneapolis VA Health Care System (MVAHCS), which is comprised of Monday through Friday each week (except federal holidays) between 7:00 a.m. and 5:00 p.m. local standard time. Place of Performance The vendor shall perform the specifications described herein onsite, at the facilities of Minneapolis VA Health Care System, One Veterans Drive, Minneapolis, MN 55417. The Job Site of the project shall be MVAHCS in the main medical center (Building 70). MVAHCS Biomedical Instrumentation Service (BIS) and Clinical staff shall define a central working location for staging equipment and grant access to the space where the equipment shall be installed on a day-by-day basis. Functional Requirements: The following functional requirements must be addressed in each vendor s proposal, providing definitive response or descriptions for each line item: Category Requirement Description System Functional Requirements See description of the system functions, requirements, expanded definitions below in Appendix A EMG Components The EMG solution shall be brand name or equal. Vendor bids shall meet or exceed the configuration, capability and function specifications of the Cadwell Industries Sierra Summit EMG systems detailed in the Bill of Material (Appendix C). In summary, the EMG solution shall be comprised of the following: 12 Channel EMG Quantity =2 each 2 Channel EMG Quantity = 2 each EMG accessories, mounting hardware, carts, and other dedicated EMG components (i.e., stimulators, electrode kits, probes, foot switches, and amplifiers) configured/required for each EMG unit Quantity = 4 kits EMG acquisition workstation hardware components (i.e., PC, Printer, monitors, EMG software/licenses, UPS, and other computing cables and accessories) for each EMG unit Quantity = 4 kits EMG Review workstation hardware components (i.e., PC, monitors, EMG software/licenses, and other computing cables and accessories) for each EMG unit Quantity = 16 EMG dedicated rack-mounted Server(s) with EMG software/licenses (up to 10 concurrent users), which includes services for server archive of all EMG users, EMG scheduling, and an HL-7 based interface to the VA s electronic medical record (CPRS/VISTA). Migration Services Clinical and Technical Training Delivery, Implementation, and Project Management Services See Bill of Materials Appendix C for additional details. Modes of Operation The EMG units shall provide the following modes of operations/measurement functions: Blink, EMG, F/H, IP analysis, MUP, NCV, RNS, SFEMG, Macro EMG, MUNE, R-R interval Evoked Potential applications shall include the following: somatosensory, auditory, visual, and cognitive Interface to the Dept of Veterans Affairs developed Clinical Procedures (CP) Application The vendor shall provide professional HL-7 programming resources to work closely with VA-provided technical resources to develop, configure, and test a functional interface EMG to the Department of Veterans Affairs developed application called Clinical Procedures (CP). Description of the interface(s) shall include but is not limited to the following: Process Flow Diagram which demonstrates the work and steps necessary to move data/information between EMG and the VA s electronic medical record (CPRS). Software required shall be provided by the vendor. The vendor shall also provide a detailed description of the network requirements with diagram of the proposed configuration. Power Requirements Line power, VAC The EMG units shall use existing electrical services of MVAMC, and connect to existing power receptacles. The system shall operate with a line power of 120 volts AC medical power and must operate at 15 amperes or less. Units shall meet UL2601 and D-IBC 60601-1 standard. The power cord shall be no shorter than 6 feet in length. It shall be coiled and storable or retractable when the unit is not plugged in. It shall be durable and able to withstand the force of being pulled from the wall receptacle while still connected to a unit as it s rolled away for use. Migration Services The vendor shall provide pricing and availability of the hardware and professional services required to migrate all historical data (including patient files, EMG data, and reports) from the existing MVAHCS-owned EMG units to the new vendor-provided solution. All migration services shall be performed on site. All removable storage device(s) containing VA sensitive data on the existing system and all removable storage device(s) onto which the VA sensitive date is transferred shall remain the property of the VA. VA sensitive data on the existing device and vendor-provided transfer device shall be cleansed and destroyed by the VA (as specified in VA Directive 6500.1). Implementation Services The vendor shall provide implementation services as part of the purchase. Implementation Services shall include the following: Project Management which shall include a detailed project timeline with defined roles and responsibilities (both vendor-provided and MVAHCS resources), on-site installation coordination of all equipment and accessories, a training plan, and schedule for Go-Live. On-site Installation Services On-site Implementation Services Other installation/implementation expenses (including potential services during off-peak times, overtime pay, and other travel expenses) Telephone/Remote Vendor Support The vendor shall provide 24/7 telephone support. The vendor support shall provide ongoing and timely resolution to all software application problems reported by MVAHCS clinical users during the warranty period. Post warranty telephone support shall be provided as part of an on-going service agreement between MVAHCS and the vendor. Trade-In The vendor shall provide trade-in values (or promotional incentives) for the following EMG equipment: Viasys - Viking II Natus/Viasys - Viking Quest See Appendix B for more information. Materials Delivery and Installer Requirement Materials provided shall be new equipment, parts, and accessories. The vendor shall deliver materials to the job site in OEM's original unopened containers, clearly labeled with the OEM's name, equipment model and serial identification numbers, and Purchase Order (PO) number. The MVAHCS shall identify and provide limited daily storage of vendor parts/supplies at the time of delivery and reject items that do not conform to this requirement. All on-site installation services shall be vendor-provided by technical staff (not sales reps) fully trained to service and install the system Disposal of Waste and Job Site Clean-up The vendor shall provide, maintain, and dispose of all material waste and packaging associated with the EMG products and/or waste generated during the installation services. Vendor will be responsible for moving waste materials from job site(s) to a vendor-provided waste container, and/or then off site. Disinfecting Instructions The vendor shall provide training instructions, procedures, and documentation necessary to properly and safely disinfect the equipment and accessories between patient uses. Technical Service Training The vendor shall provide biomedical technical training for overall maintenance and service of two (2) MVAHCS Biomed staff to include factory service training courses. If applicable, training shall include tuition, hotel, and travel expenses. Vendor shall provide any special service tools and/or test equipment comparable with that provided to the OEM s service personnel. Service and Operator Manuals The vendor shall provide the following documentation for the proposed system: Ten copies of the Operators Manual. Two copies of complete technical service manuals including troubleshooting guides, necessary diagnostic software and equipment, schematic diagrams, and parts lists. Two copies of any DVD training (and/or access to web based training). Ongoing access to clinical training materials for future staff, online or DVD On-Site Clinical Training The vendor providing the system will be responsible for training a minimum of thirty (30) clinical staff/users. The vendor prior to and at the time of installation shall provide staff education at the MVAMC. On-Site or Go-Live training shall be a minimum of 24 hours (3 x 8 hour days). A clinical education professional shall administer the training (not a sales rep). Education curriculum must include the following: Operations and set-up User maintenance Safety User troubleshooting tips Additional education materials shall include on-line (internet) resources, DVD-based, and help menus (or training aids) integrated into the EMG software. Noise in an electromyogram (EMG) recording can originate from external sources (e.g., electrode movement, power-line noise) or from internal circuit elements. The noise level shown in the chart represents that originating from internal components and is given in microvolts ( µV) peak-to-peak (p-p) or root-mean-square (rms).   CMRR @ 60 Hz, dB >115 The common-mode rejection ratio (CMRR) is a measure of the device s ability to reject power-line-frequency noise. Low rejection ratios may result in a noisy signal or wide baseline on a CRT display.   Isolated inputs Yes Input isolation is an electrical safety feature that limits the amount of current that can flow both from the unit to the patient and from another defective device through the patient and into the unit. While they increase electrical safety, isolated inputs are not required on electronystagmographs.   Input impedance, M >1,000 A measure of the electrical impedance to the signals being sensed on the body. An external input amplifier with high input impedance ensures that little current is taken from the original signal and preserves the signal s voltage. Low input impedance combined with poor electrode application may distort the electromyography (EMG) signal.
