Description

This is a combined synopsis/solicitation for commercial services prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. Network Contracting Office (NCO) 18 is issuing this combined synopsis/solicitation, VA258-17-Q-0382, as a Request for Quote (RFQ) using FAR Part 12, Acquisition of Commercial Items and FAR Part 13, Simplified Acquisition Procedures. This procurement is being set aside for small businesses. The North American Industrial Classification System (NAICS) code for this procurement is 238320, with a Small Business Administration (SBA) size standard of $15.0 Million. The Government intends award a single, Firm Fixed-Price (FFP) contract. This solicitation document and incorporated provisions/clauses are those in effect through Federal Acquisition Circular (FAC) 2005-95, effective 19 January 2017. The New Mexico VA Health Care System (NMVAHCS) has a new Acute Psyche unit that has two rooms requiring seclusion room padding in order to open the facility and occupy the space with patients. The two seclusion room dimensions are approximately 11 feet by 11 feet, 6 inches, and 9 feet in height. The contractor shall provide all products and services listed in the attached statement of work. Contract Line Item Number (CLIN) 0001: Installation of seclusion room padding in two rooms in accordance with the attached statement of work. Notice to all potential offerors: All offerors who provide good or services to the United States Federal Government must be registered in the System for Award Management (SAM), https://www.sam.gov/portal/SAM/, and completed Online Representations and Certifications (ORCA). The following FAR Clauses/Provisions apply to this acquisition: FAR 52.212-1, Instructions to Offerors Offerors must submit documentation demonstrating the following: Padding specifications that address the requirements in the Statement of Work Padding installation by persons/firm/corporation having five years experience and have completed ten successful installations of surface padding for psychiatric patients. FAR 52.212-2, Evaluation Commercial Items Award will be made to the offeror who is deemed responsible in accordance with the Federal Acquisition Regulation (FAR) and whose offer represents the lowest priced technically acceptable offer using the following evaluation criteria: Technical Capability: Padding meets specifications stated in the Statement of Work Price: No additional information from the offeror will be required if the price is based on adequate price competition. In the event adequate price competition does not exist, the contracting officer will obtain information from the offeror to the extent required to determine the reasonableness of the offered prices. Past Performance: Will be evaluated as Acceptable or Unacceptable based on past performance information obtained from any other sources available to the Government, to include, but not limited to, the Past Performance Information Retrieval System (PPIRS). In the case of an offeror without a record of relevant past performance or for whom information on past performance is not available, the offeror will not be evaluated favorably or unfavorably on past performance. Past Performance Evaluation Ratings Rating Description Acceptable: Based on the offeror's performance record, the Government has a reasonable expectation that the offeror will successfully perform the required effort, or the offeror's performance record is unknown. Unacceptable: Based on the offeror's performance record, the Government has no reasonable expectation that the offeror will be able to successfully perform the required effort.
