Description
Amendment No. One (0001) is hereby issued to add training to the SOW and answers to questions asked about the project as follows: 1. Answers to questions asked are as follows: Since the property is located in Utah, will you, your organization and the end user Uintah & Ouray Service Unit, Ft Duchesne Health CTR, Fort Duchesne, Utah, accept and allow a CA Low Voltage contractor perform this work in Utah without any UTAH contractors license or without approval from a UTAH state contractors agency to conduct work in Utah? The selected contractor has to meet the requirement of the solicitation. ANSWER: The solicitation does not mention a contractor license is a requirement. As long as they can successfully perform what if required by the Scope of Work (SOW). The "Provider" we will offer in our bid is a worldwide telecommunication phone carrier/"Provider", providing telecom service and phone lines to business' in most states in America, has the highest security rating of any "Provider" I have found (AT&T, Verizon, Sprint, Charter, Spectrum, etc.), higher fail safe rating 99.9% uninterruption of service, but has no special approvals to offer telecommunication VoIP or other analog phone lines in Utah. Is this acceptable to your agency, Utah and the end user Uintah & Ouray Service Unit, Ft Duchesne Health CTR, Fort Duchesne, Utah? ANSWER: I'm fine with it so long as having a non-local provider service doesn't cause us problems in paying bills or getting things fixed quickly when there are problems as there surely will be from time to time. We can't afford to have our lines down for more than a few hours up to maybe a day at a time. If it will take a non-local provider longer than this to fix problems, it won't meet our needs. Also, just keep in mind that this solicitation is a 100% Small Business Set-Aside so AT&T, Verizon, Sprint, will not qualify because they are not a Small Business. How many hours of training are you requesting to be included in this bid by contractor and "Provider"? ANSWER: 2 Hours of training will be adequate. Is there a site visit requirement before bidding? ANSWER: No May we schedule a site visit before the RFI period ends on May 15, 2017? If yes, what documentation do we need to provide to gain access on site, when do we need to provide this information and whom do we send the information to? ANSWER: We have determined that there will not be a site visit due to the simplicity of the project. Being this this project is simple phone installation and cable drops we will not have a site visit. We can provide more information to you. Just let us know what type of information you need. The project is just a simple telephone installation and cable drops plus providing telephone and internet services. Is there a range of bid amounts normally provided on other RFP/ for this RFQ? The solicitation is not based on price but based on best value to the Government that include price quote, past performance and technical proposal. ANSWER: Evaluations will be done to rate each vendors solicitation documentation submitted. The vendor that provides the overall best value to the Government shall be awarded the contract. 2. Add Two (2) Hours of Training to the Scope of Work as follows: The Contractor shall provide Two (2) Hours of Training on the newly installed telephones. 3.All Other Terms and Conditions Remain Unchanged.
