(ii) Contract number, type, date, name of project; (iii) Description of the work; (iv) Initial Contract amount and final contract amount; and (v) Name, telephone number, and e-mail address of a point of contact for the customer that the Government may contact to confirm and verify the information provided. The Government may not evaluate references with incorrect or missing phone numbers and contact names, which may potentially result in a less favorable rating of the contractor ¿s offer. The Government may also use information obtained from other than the sources identified by the offeror. Information obtained from the Past Performance Information Retrieval System (PPIRS) database located at http://www.ppirs.gov is one of the sources that will be utilized. PPIRS functions as the central warehouse for performance assessment reports received from several Federal performance information collection systems and is sponsored by the DOD E-Business Office and administered by the Naval Sea Logistics Center Detachment Portsmouth. (3) Past Performance - Offerors shall have a Past Performance Questionnaire (PPQ), attached to this solicitation, completed by no more than (3) of the offeror ¿s previous clients/owners. Offerors are strongly encouraged to inform previous clients of the importance of the questionnaires as it is a determining factor in rating the offer. It is the offeror ¿s responsibility to ensure that the government receives the requested questionnaires by the time and date specified in the solicitation for the receipt of quotes. The Government will also use past performance information obtained by other sources than those identified by the offeror. The Government will utilize the Past Performance Information Retrieval System (PPIRS) database located at http://www.ppirs.gov for obtaining past performance information. Offerors will be evaluated on the degree to which they have completed similar projects on schedule and without performance problems. Information submitted regarding adverse performance problems will be considered, as well as mitigating circumstances detailed in the proposal for any performance problems identified. (4) Price - The Government will consider quotations for award only for those offers that have provided pricing for all listed tests, exams, procedures, and administrative fees. In order for the Government to consider a quotation for award, all listed tests, exams, procedures, and administrative fees must have a price listed. All proposed prices will be evaluated for price reasonableness and any incidents of unbalanced pricing. (b) Options. The Government will evaluate offers for award purposes by adding the total price for all options to the total price for the basic requirement. The Government may determine that an offer is unacceptable if the option prices are significantly unbalanced. Evaluation of options shall not obligate the Government to exercise the option(s). (c) A written notice of award or acceptance of an offer, mailed or otherwise furnished to the successful offeror within the time for acceptance specified in the offer, shall result in a binding contract without further action by either party. Before the offer ¿s specified expiration time, the Government may accept an offer (or part of an offer), whether or not there are negotiations after its receipt, unless a written notice of withdrawal is received before award. Session Timeout Warning {1} -------------------------------------------------------------------------------- THIS IS AN AMENDED PRESOLICITATION NOTICE TO UPDATED THE SOLICITATION ISSUE DATE TO "ON OR AROUND MAY 1, 2017" AND THE QUOTE DUE DATE TO "JUNE 1, 2017" THIS IS NOT A SOLICITATION FOR QUOTES. THIS IS A SYNOPSIS OF AN UPCOMING SOLICITATION The Bureau of Reclamation, Great Plains Region intends to solicit quotations for medical surveillance program services throughout the entire Great Plains Region. Services include, but are not limited to: coordinating new and periodic evaluations, ensuring proper testing has been completed, completing Reclamation specific documents. The Great Plains Region encompasses all or parts of nine states including Montana, North Dakota, South Dakota, Wyoming, Colorado, Nebraska, Kansas, Oklahoma, and Texas. It is emphasized that coverage of the entire Great Plains Region within 100 miles of an employee's place of employment is a requirement for this contract. The largest areas include, but are not limited to Loveland, Colorado; Casper, Wyoming; Billings, Montana. Although few, some pre-placement exams maybe needed outside of the Great Plains Region. Pursuant to Part 12 and Part 13 of the Federal Acquisition Regulations, Reclamation intends to issue a Request for Quotation (RFQ) on or around May 1, 2017. Quotes will be due on or around June 1, 2017. Reclamation anticipates issuing the RFQ set-aside for small business. The applicable North American Industry Classification System (NAICS) Code is 621111 and the associated Small Business Size Standard is $11.0 million. The Government anticipates award of a fixed price Indefinite Delivery Indefinite Quantity (IDIQ) in July 2017. Reclamation will consider all responsible sources who submit a timely quote that conforms to the solicitation. Prospective offerors may submit quotes through the mail, electronically, e-mail, or fax. The resulting IDIQ contract will include a base 1-year period, and up to 4 1-year option periods. Any contractor interested in doing business with the Federal Government must be registered and active in the System for Award Management database to be considered for award. SAM is a Federal Government owned and operated FREE website at www.SAM.gov. All invoices for this procurement must be submitted through the Department of Treasury's Internet Payment Platform located at www.IPP.gov.
