This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. The solicitation number is VA250-17-Q-0591 and is issued as a request for quotation (RFQ) in accordance with FAR Parts 12 and 13. The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-95. The associated NAICS code is 339112 Surgical and Medical Instrument Manufacturing and small business size standard is 1,000 employees. This is a total service disabled veteran owned small business set-aside. The requirement is for (1) Zeiss OS Cirrus HD Oct (Model 5000) for use at the Louis Stokes VA Medical Center, 10701 East Boulevard, Cleveland OH 44106 This announcement constitutes the only solicitation; a formal, written solicitation will not be issued. PAPER COPIES OF THIS SOLICITATION WILL NOT BE AVAILABLE. This solicitation documents and incorporates provisions and clauses in effect through FAC 2005-95. It is the responsibility of the contractor to be familiar with the applicable clauses and provisions. The clauses may be accessed in full text at these addresses: https://www.acquisition.gov/Far/. The selected Offeror shall comply with the following commercial item terms and conditions, which are incorporated herein by reference: FAR 52.212-1, Instructions to Offerors -Commercial Items, FAR 52.212-3, Offeror Representations and Certifications-Commercial Items - the selected offeror shall submit a completed copy of the listed representations and certifications or confirm that the annual representations and certifications was completed electronically via the SAM website accessed through http://www.acquisition.gov; FAR 52.212-4, Contract Terms and Conditions-Commercial Items; 52.203-17, 52.203-99, and 52.204-4. Contract Title: ZEISS CIRRUS HD-OCT PRINTER Scope: The Louis Stokes Cleveland VAMC is requesting the following items for Ophthalmology and Optometry Clinics located at the main Medical Center: Item # Description/Part Number* Qty Price Extended Amount 1 Zeiss OS; Cirrus HD-OCT (Model 5000) 1   Total   Specific Task: 3.1 Task 1 Delivery of Equipment and Supplies The vendor will deliver the above items to the Cleveland VAMC within 1-2 weeks or order placement Deliverables: The equipment will be delivered to the facility: Louis Stokes Cleveland VA Medical Center 10701 East Boulevard Cleveland, OH 44106 3.2 Task 2. Installation and Assembly Insulation will be provided by the vendor/ manufacturer. Deliverables: Installation and assembly of equipment will be complete and approved for use by the vendor, the medical staff, biomed, and any other applicable groups. 3.3 Task 3. Warranty A one year warranty is offered by the manufacturer. Performance Monitoring: The COR will monitor the successful delivery of the equipment by maintaining contact with the vendor and coordinating with the VA Warehouse and Biomedical Engineering personnel. The COR will also ensure the equipment is assembled and all installation of hardware and software is done to contract specifications Security Requirements: The equipment and accessories will not connect to the VA network. Government Furnished Equipment (GFE)/Government Furnished Information (GFI): There will be no Government-Furnished Equipment or Government furnished Information provided to the vendor. Other Pertinent Information or Special Considerations: Identification of Possible Follow-on Work. Possible follow-on work could be contact with the Vendor for troubleshooting problems. Also, vendor may be notified if the product is not functioning properly Identification of Potential Conflicts of Interest (COI). There are no known potential conflicts of interest. Identification of Non-Disclosure Requirements. The vendor will not have access to any VA patient sensitive information or proprietary information. Packaging and Shipping Instructions: Packaging and shipping will be at the discretion of the vendor Inspection and Acceptance Criteria: Products will be inspected upon arrival by the COR and the Surgical Service Risk Control: The Surgical Service will follow any safety recommendations given by the vendor for the equipment. Place of Performance: The equipment will be used in the Parma Optometry Clinic.
