- Attachment 4: GPAT Attachment 3: Cloud Questionnaire Attachment 2: Past Performance Part II Attachment 1: Past Performance Part I Full Copy of Solicitation/RFQ# HE1254017-Q-0009 The Department of Defense Education Activity (DoDEA) intends to award a firm-fixed unit price requirements contract to fulfill its requirement for Comprehensive Web-Based Support for Teachers of K-12 Students with Moderate-to-Severe Disabilities using Federal Acquisition Regulation (FAR) parts 12 and 13 procedures. This opportunity is 100% set-aside for small businesses. The award will be evaluated using best value trade-off procedures. This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. The solicitation document, and incorporated provisions and clauses, are those in effect through Federal Acquisition Circular 2005-95 and Defense FAR Supplement Publication Notice (DPN) 20161222. The Solicitation# is HE1254-17-Q-0009, is issued as a Request for Quote (RFQ), and is 100% set-aside for Small Businesses. The period of performance is one 12-month base term followed by four consecutive 12-month option terms (overall 5 years). The anticipated award date is late July 2017. In order to be eligible for award Offerors have to be registered with an active profile in the System for Award Management (SAM) website at www.sam.gov. DoDEA is a federal government agency, an education activity, tax exempt, and qualifies for academic/educational pricing. No information concerning this solicitation or request for clarifications will be provided in response to telephone calls. All timely inquiries received via email to nina.lin@hq.dodea.edu with subject line "K-5 Science RFP Questions" will be answered via an amendment to the solicitation. Offerors are responsible for downloading the solicitation, all supporting documentation, and any applicable amendments posted on Federal Business Opportunities (FedBizOpps). Amendments may be posted to FedBizOpps until the response due date. Please note that there are multiple due dates for different portions of the response.
