Description

This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. The solicitation number is VA259-17-Q-0521 and is issued as a request for quotation (RFQ). The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-95. The associated NAICS code is 333314 Optical Instrument and Lens Manufacturing and small business size standard is 500 employees. This is a total small business set-aside. The requirement is for a Biometer to specifically be utilized for intraocular lens calculations. IOL MASTER 700 WITH swept (or Equal) for use at the PFC Floyd K. Lindstrom Clinic at Colorado Springs. Delivery location is the PFC Floyd K. Lindstrom Clinic at Colorado Springs, 3141 Centennial Boulevard, Colorado Springs, CO 80907. This announcement constitutes the only solicitation; a formal, written solicitation will not be issued. PAPER COPIES OF THIS SOLICITATION WILL NOT BE AVAILABLE. This solicitation documents and incorporates provisions and clauses in effect through FAC 2005-88. Failure to do so will result in an unacceptable offer. A proposed delivery/installation schedule is to be provided.
