Description

This is a Combined Synopsis/Solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only Solicitation; quotes are being requested, and a written Solicitation will not be issued. Solicitation number VA248-17-Q-0671 is issued as a Request for Quotation (RFQ). The attached solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular (FAC) 2005-94, 2005-95-1 dated 01/19/2017. Responses should be sent to Shirley.Courtney@va.gov. NAICS and Set-aside: The NAICS is 339113 and the small business size standard is 750 employees. This procurement is set-aside for Service-Disabled Veteran-Owned Small Businesses and Veteran Own Small Businesses (SDVOSB/VOSBs). To receive consideration as SDVOSB/VOSB s the firm must be registered and verified in the VIP database at www.vip.vetbiz.gov. Also, to receive a government award, the contractor must be currently registered in System for Award Management (SAM) at www.sam.gov. The government anticipates awarding a Firm-fixed price contract. An award will be made to Lowest Price quote, which conforms to the requirements within this solicitation and represents the best price to the government. Description of Supplies requested: The Veterans Health Administration, Network Contracting Office 08, North Florida/South Georgia, Veterans Health System, requires the following supplies: PAPR System (Powered Air Purifying Respirator), S-5000-04 with Alkaline battery pack S-4013. Submitting Quote: Contractor shall submit their quote on company letterhead and shall include annual cost with the price/cost schedule (See attached Price/Cost Schedule); any offered discounts; name, address, and telephone number of the offeror; terms of any express warranty; completed copy of 52.212-3 Offerors Representations and Certifications- Commercial Items. Interested contractors must submit a copy of any relevant certifications and licenses along with their quote. Quotes must be received May 25 2017 by 12:00 PM EST. All quotes must be submitted by electronic submission in reference to VA248-17-Q-0931 to Shirley.Courtney@va.gov. Quotes received after the exact time specified in the solicitation are considered late and may be considered if the action would not unduly delay the acquisition. All questions regarding this solicitation must be submitted to the Contracting Officer in writing (email is preferred).
