- Request for Quote Matrixmandible implant and instrument set - best and final offers. 3. The DDHU Stock Maintenance Division, Hill AFB, UT has the following requirement: Item: Matrixmandible implant and instrument set MFG Part Number: Synthes CMF 01.503.150 National Stock Number (NSN): 6515-01-644-7977 Quantity: 2 Each This action is conducted under the Multiple Award Schedule Program. The authority waiving the competition requirement is FAR 8.405-6 (b)(1) - only one manufacturing source is capable of providing the supplies or services required at the level of quality required because the supplies or services ordered are unique or highly specialized. The mandibular plating system is a specialized surgical set that is a required and critical component within the Army's Dental Materiel Set Oral: Maxillofacial Surgery unit assemblages. It is used by deployable forces and contingency operations by surgeons to perform specialized trauma surgery. This item was confirmed a requirement by the U.S. Army Medical Department Center and School (USAMEDDC & S), Health Readiness Center of Excellence (HRCoE) and Capabilities Development Integration Directorate (CDID) during scheduled medical set reviews. It is a required item in all Army Dental Material Sets to support deployed medical Combat Support Hospitals. It is also a commonly used set for surgical facial reconstruction in both fixed and field hospitals and, for training purposes at the Army's Pre-Deployment Regional Medical Training sites. Its weight, size and cube conforms to meet the standards and fits within larger surgical medical sets and the set container fits within the Army current field surgical sterilizers. These mandible sets are required for the 10th Field Hospital, Fort Carson, Colorado and the 405th Field Hospital, in care of the Sierra Army Depot, Medical Materiel Readiness Program Office of the Surgeon General, California. The mandible plating set is an enduring requirement approved by the U.S. Army CDID. These sets are an authorized requirement for Army medical unit assemblages (UA): P477, Dental Materiel Set Oral: Maxillofacial Surgery. The minimum clinical performance requirements: - Size and cube meeting the unit assemblage requirements by USAMEDDC&S, HRCoE and CDID. - All components of the approved 6515-01-644-7977 NSN. - All components are manually operated and do not require an electrical power source. - Food and Drug Administration certification. 4. No substitute or waiver of the primary items or accessory components will be accepted for this solicitation. 5. This buy is a logical needs requirement based on the fielding scheduled for the Army's programmed transition of current Combat Support Hospital configuration to the approved Field Hospital configuration. This transition is scheduled to commence through 2022. The conversion process will affect Active Duty, National Guard and U.S. Reserve authorized units.
