Failure to provide the requested information to help determine specific capabilities may affect the manner in which the service is solicited. Simply providing a response showing interest or capability without proving information and / or responses to specifically requested information, WILL be deemed non responsive. Page 1 of Page 2 of 11 Request for Information/Sources Sought THIS REQUEST FOR INFORMATION (RFI) /SOURCES SOUGHT (SS) NOTICE IS NOT A REQUEST FOR PROPOSAL (RFP). This is a market research tool IAW (FAR Part 10) to determine potential firms and their capability to provide the services requested / described herein. The information received will be used to determine the method of acquisition. Feedback is encouraged and appreciated. All small business concerns are encouraged to respond. The VA Maryland Healthcare System is seeking information on potential sources to provide all labor, supervision and management for the following: Hazardous, Radiological and Universal Waste Characterization, Identification, Management, Removal, Disposal, and associated emergency response. All of these services must be in compliance with all applicable State and Federal environmental laws. Please provide a statement or response that answers in detail, the following questions: ** Business name and / or DBA name: _________________________________________ Small Business Type (if applicable): ___________________________________________ (i.e. SDVOSB, VOSB, 8a, WOSB, SB etc.) GSA Contract # (if applicable): ______________________________________________ Duns #: _______________________________________________ Primary NAICS: ________________________________________ Primary PCS: __________________________________________ How many years has your organization performed US Environmental Protection Agency (EPA)/Maryland Department of the Environment (MDE) Resource Conservation and Recovery Act (RCRA) hazardous waste identification and hazardous waste Characterization (to include sampling and testing in accordance with appropriate test methods such as EPA SW-846 and/or other legally-required test methods)? Would you please describe your process? Have you (or a subcontractor working for you) performed RCRA Characterization of a pharmacy formulary? If this task is subcontracted, please state. Do you provide Waste profiles for each of the waste streams that you manage and dispose of at a healthcare facility? Please describe. How has your organization demonstrated knowledge in the Maryland State regulations governing hazardous waste generation, treatment, storage, transportation and disposal? How many EPA/MDE RCRA Hazardous Waste Satellite Accumulation Areas (SAA) have you managed within one healthcare system? Specifically, from how many SAAs have you picked up waste and transported it to the healthcare system s RCRA hazardous waste less than 90 day central accumulation storage area and/or offsite to a Treatment, Storage, or Disposal Facility (TSDF)? Would you please describe your process as well? How many EPA/MDE RCRA Universal Waste Accumulation Areas have you managed within one healthcare system? Specifically, from how many universal waste accumulation areas have you picked up waste and transported it to the healthcare system s RCRA hazardous waste less than 90 day central accumulation storage area and/or offsite to a universal waste recycling facility? Would you please describe your process as well? How many years have you (or a subcontractor working for you) provided radiological waste management and disposal for a healthcare facility? Please also describe your process. If this task is subcontracted, please state. Do you provide lab-pack services? If so, would you please describe your process? Have you prepped and lab-packed chemicals from lab decommissioning for hazardous waste shipment in the past five years? If so, provide an example. How many years experience do you have in providing RCRA Recordkeeping/Reports and Technical Assistance to a healthcare organization such as RCRA Biennial Reports? Does your organization hold MD state Hazardous and Universal Waste hauler licenses? If so, for how many years? Have your customers visited one or all of the TSDFs where their hazardous wastes are shipped for final waste disposal? How many years have you (or a subcontractor working for you) provided Emergency HAZMAT and/or hazardous waste spill response? Please describe. If this task is subcontracted, please state. Do you (or a subcontractor working for you) have experience providing annual DOT RCRA training? If this task is subcontracted, please state.
