All work shall be performed in accordance with the USCG specifications, provisions, clauses and the terms and conditions which will be provided when all Request for Quote (RFQ) solicitation document files become available for public download on or after 15 May 2017 exclusively from the internet by accessing the Federal Business Opportunities (FBO) website at: http://www.fbo.gov. If solicitation files do not appear on that date, please check back daily. The date set for receipt of offers is 16 June 2017 at 1400 EDT. This procurement is 100% SET-ASIDE FOR SMALL BUSINESS CONCERNS SCOPE OF WORK HSCG 42 17 QMAA09 GENERAL REQUIREMENTS: The work consists of providing all labor, equipment, material, transportation, and supervision as required to provide monthly pest control services at the following Coast Guard units: Sector Delaware Bay, Station Indian River, Station Barnegat Light, Station Manasquan, Station (sm) Shark River, Station Atlantic City, Station (sm) Great Egg, Station (sm) Townsend Inlet, Station (sm) Fortesque. Price to be provided for base year plus 4 option years. Price should include emergency service work based on hourly rate with service call fee. Materials to be provided for emergency services are to be provided at best possible cost to government. Emergency service shall be provided within 48 hours, when requested. All USCG units and, addresses, and Pest Control Managers (PCM) will be listed in enclosure (1). 2. COMMENCEMENT, PROSECUTION, AND COMPLETION OF WORK: Upon acceptance, the contractor shall commence once-a-month service starting 01 Oct 2017 and ending 31 Sep 2018. Contractor shall schedule all preventative maintenance visits with PCM for each unit. 3. SCOPE OF WORK: Provide pest control services for nuisance pests (ants, roaches, etc). These services shall be on a scheduled matrix to prevent these pests on the Coast Guard property. Mechanical traps and baits may also be used at the discretion of the servicing technician. Material Safety Data Sheets (MSDS) shall be provided for all products that will potentially be applied to property. 3.a. The following services will be provided to the Coast Guard units listed below on the following schedule: Monthly: The mess deck and all food handling areas of the galley are to be treated for nuisance pests. This is to include any service elevator and/or stairwells located at the galley. Semi-Annually: All berthing-area rooms are to be treated for nuisance pests. This service shall be scheduled and completed in the Spring and in the Fall. Annually: Buildings will be treated for nuisance pests inside and around the perimeter of the buildings. This is to include all common areas, shop spaces, elevators, stairwells, and exterior to include termite inspection on applicable buildings. This service shall be scheduled and completed in the Summer. SITE VISIT: All bidders are strongly encouraged to visit the site to verify any/all quantities before submitting bids. Interested parties shall make arrangements with each Coast Guard Unit's PCM listed in Enclosure (1). THERE WILL BE NO GOVERNMENT SCHEDULED SITE VISITS. 5. DESIGNATED PEST CONTROL MANAGER (PCM): The PCM at each unit will be the vendor's point of contact for scheduling work. The PCM will notify the Contracting Officer of any requests outside this scope from the vendor. 6. CONDITIONS AFFECTING THE WORK: The Offeror should visit the site and take other steps as may be reasonably necessary to ascertain the nature and location of the work, and the general and local conditions which could affect the cost of the work thereof. Failure to do so will not relieve bidders from responsibility for estimating properly the difficulty or cost of successfully performing the work. The Government will assume no responsibility for any understanding or misrepresentations concerning conditions made by any of its officers or agents prior to the execution of the contract, unless included in the request for quotes, the specification, or related documents. 7. AFTER AWARD OF THE CONTRACT: The Contractor must immediately notify the Contracting Officer's PCM of his intended start date. The work shall be done in accordance with the specification. Pest Control Applicators MUST be licensed in the state where work is performed and must also comply with any local regulations (county, city, etc.). 8. ORAL MODIFICATION: No oral statement of any person other than the Contracting Officer shall in any manner or degree, modify or otherwise affect the terms of this contract. 9. WORKMANSHIP: All work shall be accomplished with the best standard practices and by workmen thoroughly experienced in the required trades. The work shall be supervised by experienced personnel and shall be satisfactory to the Contracting Officer. 10. PERMITS: The Contractor shall without additional expense to the Government, obtain all appointments, licenses, and permits required for the prosecution of the work. The Contractor shall comply with all applicable federal, state, and local laws. 11. CLEAN UP: All trash and debris shall be removed from the site daily. The job site shall be left in a clean and neat manner. 12. WORK OUTSIDE REGULAR HOURS: If the Contractor desires to work on Saturday, Sunday, holidays, or outside the unit's regular hours, the Contractor must submit their request in writing to the Contracting Officer's PCM for approval consideration. The Contractor shall allow ample time to enable satisfactory arrangements to be made by the Government for inspecting the work in progress. 13. UPON COMPLETION OF WORK: Contact the Contracting Officer's PCM to arrange for inspection/acceptance of work. 14. IDENTIFICATION OF CONTRACTOR PERSONNEL AND VEHICLES: The Contractor shall provide a detailed list of all employees to the Contracting Officer five (5) days prior to commencing work. The employee list shall contain the employee's full name, date, place of birth, current address, and last four (4) of SSN. Coast Guard Unit's PCM will be given a copy of the employee list. Each Contractor-provided vehicle or towed trailer shall show the Contractor's name so that it is clearly visible on the vehicle. A valid state license plate and safety inspection sticker is required. All vehicles operated on Government property shall be maintained in a good state of repair. 15. SAFETY REQUIREMENTS: The Contractor shall conform to all state and local safety requirements and those of each Coast Guard Unit. The Contractor and their employees shall become familiar with and obey all regulations including fire, traffic, and security regulations. There will be no smoking in Coast Guard Buildings. No underage visitors authorized onsite. Employees and suppliers only, no dependants, minors, or anyone else not directly associated with the ongoing work are authorized onsite. No pets or animals of any kind allowed on worksite. The exception being service animals for the visually or hearing impaired. Documentation will be required for the animal to remain on site. 1. Responsibilities: a. All Contractors working at each of the Coast Guard Units shall abide by all federal safety regulations as published by the Occupational Health and Safety Administration, 29 CFR 1910 (series), 29 CFR 1925 (series), and 29 CFR 1926 (series). Contractor's written safety programs shall be reviewed by the respective Coast Guard Unit's PCM prior to starting work. b. Written notification must be provided to the Coast Guard Unit's PCM in their capacity as Fire Marshal of any activity that could potentially cause fire or explosion or that could change or reduce the capability of fighting a fire or explosion. Some examples of qualifying activities are: a fire hydrant or fire suppression system that must be taken out of service for any reason, electrical work that affects fire alarm systems, introduction of hazardous or flammable material onto the unit, and blocking of any egress routes or emergency vehicle routes. 2. Rights: a. Every employee working on Coast Guard property has the right to a safe and healthy work place. The Contractor has the responsibility and right to stop unsafe work. PCM's and the Coast Guard Unit Safety Officer may also institute a work stoppage relating to unsafe practices or immediate danger to life and health situations. 16. ENVIRONMENTAL PROTECTION REQUIREMENTS: a) All Contractors working on any Coast Guard Units shall abide by all federal environmental regulations as published by the Environmental Protection Agency, 40 CFR (series). Contractor written environmental management and pollution prevention programs shall be reviewed by the Coast Guard Unit's PCM, prior to starting work and must be in accordance with the Unit Environmental Guide. b) Written notification must be provided to the Coast Guard Unit's PCM of any activity that could potentially cause a permit violation on any Coast Guard Units listed. Some examples of qualifying activities are: Any activity that could result in a spill or discharge into a waterway or introducing a hazardous material into the environment. c) Copies of the following must be provided to the Coast Guard Unit's PCM prior to starting work in affected areas: Material Safety Data Sheets (MSDS), Permits, and notices of intent. d) Hazardous materials used by a Contractor are required to be disposed of by that Contractor. Disposal must be in accordance with federal, state, and local guidelines. e) This maintenance is to include monitoring, identification, inspection, and lastly chemical treatment to prevent and eradicate general pests, not to include wildlife, or wood boring /blood sucking pests. Monitoring shall be done by utilizing snap, glue, bait houses, or other non-chemical traps to identify specific pests in each location. Visual inspections of each trap and overall location shall be done prior to treatment to ensure the most effective treatment protocol is being used; chemical treatment includes spraying, insecticide pellets, etc. Contractor shall operate in a manner that minimizes risk of contamination to the environment and personnel. All pesticide application from contractors will comply with federal, state, and local regulation. Contractor may not use any EPA "Restricted Use" chemicals. All pesticides must be brought in for each service and contractor may not store extra materials at the individual units. f) When pest control services are completed, the technician shall submit a copy of all paperwork to minimally include; name of applicator, chemical used, quantity used, date of application, location, targeted pest, and description of the pest problem to the PCM. Contractor shall be licensed and certified in the state of performance and submit a copy of the license and certification.
