- RFQ Request for Quote: Urine Drug Screen System Prospective vendors shall be properly registered in www.sam.gov. This includes proper registration as a small business including subcategory claimed such as HUBZone, 8(a) SDVOSB, WO in order for IHS to issue an award and claim procurement actions with eligible small business vendors. Offers shall be received on or before the closing date and time listed. Offers shall be legible in order to be considered. Facsimile is not acceptable. E-mails: E-mailed attachments shall be in a printer friendly format using 8"x11" page size PDF. All E-mails (Q&A, Quotes, Amendments) shall include a proper e-mail subject line which identifies the solicitation number (17-239-SOL-00026) at the front of the subject line. Failure to properly title an e-mail may result in removal due to spam identification. Quotation shall include pricing for all years: One (12) month base year plus four (12) month option years. Include Dun & Bradstreet Number with offer.
