- COMBINED SYNOPSIS/SOLICITATION Karl Storz Sinus Telescope (I) This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice and in accordance with the simplified acquisition procedures authorized in FAR Part 13. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. (II) This solicitation is issued as a request for quotation (RFQ). Submit written quotes on RFQ Number: VA256-17-Q-0728. (III) The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-87. (IV) This solicitation is being issued as a Total Service Disabled Veteran Owned Small Business Set-Aside. The associated NAICS code is 339112. The small business size standard is 1000 Employees. (V) This combined solicitation/synopsis is for purchase of the following commercial services: Contractor shall furnish Karl Storz Sinus Telescope for the Houston, VAMC. (VI) Description of requirements is as follows: Contractor shall furnish Karl Storz Sinus Telescope including the following listed items for patient care: LINE ITEM QUANITY UNIT OF ISSUE 7230BVA Hopkins II 30 ° Telescope, Reverse 4 EA Light Post, 4mm x 18cm, Autoclavable 7230FVA Hopkins II 45 ° Telescope, Reverse 4 EA Light Post, 4mm x 18cm, Autoclavable KSZ-39301AS-ZBD Container System for two telescopes 4 EA to 4mm x 18cm, Steam/Sterrad, 12 x6 x3 THIS IS A BRAND NAME SPECIFIC REQUEST, NO OTHER PRODUCT WILL BE ACCEPTED. Please do not submit a response to this solicitation unless you are offering the specific requested equipment. Quotes with Brand Name Equal Equipment will be rejected. (b) The place of acceptance will be: 2002 Holcombe Blvd, Houston, Texas 77030 Quotes are required to be received in the contracting office no later than 11:30 A.M. CST on May 22, 2017.
