Re: Optical Coherence Tomography (OCT) Introduction The General Services Administration (GSA) is considering a new requirement to provide an optical coherence tomography (OCT) to the VA Hospital located in Ft. Harrison, Montana. The Government does not intend to award a contract on the basis of this Request for Information (RFI). The purpose of this RFI is to gather data and current market place information from industry, including small business firms. The information obtained from this RFI will be used to assist in developing the most suitable acquisition strategy. Furthermore, interested parties will not be reimbursed for any costs related to providing information in response to this RFI. The proposed North American Industry Classification System (NAICS) code for these services is 423450, Medical, Dental, and Hospital Equipment and Supplies Merchant Wholesalers, with a small business size of 200 people. This RFI is meant to achieve the following goals: 1.) To survey the market place to determine what type of firms are available to provide this equipment. A. General Description of the Services Requested Two Optical Coherence Tomography (OCT) machines are requested. Optical Coherence Tomography (OCT) is a commonly performed diagnostic test designed to assist the doctor in identifying retinal diseases, such as age-related macular degeneration (AMD) or diabetic retinopathy (diabetic eye disease). With an optical coherence tomography (OCT) we can look closely at the retina and obtain a virtual, non-invasive biopsy. Similar to a CAT scan, the OCT uses infrared light rather than x-rays to form a cross-sectional view of the retina. This detailed image of the retina allows the doctor to see the retina at a microscopic level and detect swelling in the retina. Responses from Industry Responses and input from interested parties are requested. In responding to this RFI please provide the following information: · A capability from your company demonstrating that you can provide these supplies, and provide delivery to Ft. Harrison, Montana. · Answer all of the questions below. Specifically answering your socio-economic status, if your company has a Federal Supply Schedule, and if your company is on schedule can all of the equipment be procured utilizing that schedule. · Response to the following questions limiting your total response to up to 4 pages (8.5”x11”, Times New Roman font, size 12). C. Questions for Industry 1.) What is your company name, address, point of contact, telephone number, email address, and size of your organization (ie large business, small business) using NAICS 423450, as the basis of your response. 2.) Are you a small business? If yes, what socio-economic category does your business identify as? (Hubzone, 8A, Women Owned Small Business, Veteran Owned Small Business, etc.). 3.) Is your company a VA Certified Service Disabled Veteran Owned Small Business (SDVOSB) or Veteran Owned Small Business (VOSB)? 4.) Are you able to provide equipment that is compliant with the Buy American Act? 5.) How quickly can you deliver this type of equipment? 6.) Do you have a Federal Supply Schedule Contract? If yes, is all of the equipment listed available on your schedule? 7.) Is this equipment available from an American manufacturer? If no, then what would be the country of origin for this type of medical equipment? D. Response Requested Your response must be provided no later than 10:00 AM MST Friday, May 23, 2017.
