Description
This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6, in conjunction with FAR Part 13; and supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. The solicitation number is VA262-17-Q-0958 is issued as a request for quote. The North American Industry Classification (NAICS) Code for this acquisition is 811310, with a business size standard of $7.5 Million. An award will be made to the lowest priced technically acceptable quote. Please identify your business and supplemented with additional information included in this notice. This requirement is put forth as Unrestricted. The VA Loma Linda Healthcare Center seeks to issue a purchases order for service and maintenance for a Champion Dish Machine and Conveyer. The Contractor shall furnish all materials/services at the Department of Veterans Affairs, Loma Linda Healthcare System, 11201 Benton Street, Loma Linda, CA 92357. Request service for item listed below: The Department of Veterans Affairs (VA), VISN 22 Network Contracting Office, is seeking sources that can provide service for the Champion Dish Machine and Conveyer all of which meets the following requirement in accordance with the Statement of Work for the VA Loma Linda Healthcare System: List of attachments 1. Statement of Work The information identified above is intended to be descriptive, not restrictive and to indicate the quality of the supplies/services that will be satisfactory. It is the responsibility of the interested source to demonstrate to the government that the interested parties can provide the supplies/services that fulfill the required specifications. STATEMENT OF WORK DEPARTMENT OF VETERANS AFFAIRS Loma Linda Service and Maintenance for Champion Dish Machine and Conveyor GENERAL OBJECTIVES AND REQUIREMENTS General: This project is intended to perform annual service and maintenance on the CHAMPION dish machine and conveyor located at the Department of Veterans Affairs Loma Linda Healthcare System, 11201 Benton St., Loma Linda CA 92357 Description of Services/Scope of Work: Contractor shall provide all necessary labor, transportation, equipment, materials, personal protective equipment, supervision, other items and non-personal services required to provide the annual maintenance and testing on dish machine and conveyor described in the Statement of Work for the Department of Veterans Affairs Loma Linda. Physical Security: The Contractor shall be responsible for safeguarding all government equipment, information and property provided to the Contractor personnel while performing the services described herein. Period of Performance: The period of performance shall be for a (365) thirty-day period. The period of performance shall be for one (1) Base Year of 12 months and three (3) 12-month option years. The Period of Performance reads as follows: The total period of performance of this contract is for four (4) years; which includes a one (1) year base period and three (3) - 12-month option years. Each period of performance must be 12 months. If the government elects to exercise any of the option years, written notification of the Government's intent to exercise such option will be provided to the contractor at least 30 calendar days before the contract expiration date. Contract period of performance schedule is as follows: Base Period: from date of contract award, not to exceed 12 months. 01 June 2017 through 31 May 2018 Option Years: Option Year 2 -1 June 2019 through 31 May 2020 Option Year 3 - 1 June 2020 through 31 May 2021 Type of Contract: The government will award a Firm-Fixed Price (FFP) contract. Designated Point of Contact: The Designated Point of Contact will provide unlimited communication with the Contractor in response to the service and maintenance of the CHAMPION dish machine. The Designated Point of Contact shall not provide any formal direction and/or guidance to the Contractor on the performance of the CHAMPION dish machine. The Designated Point of Contact will be on the premises where the services will be performed. Contract Administration: The Contracting Officer is the only person authorized to approve changes or modify any of the requirements under this contract. The Contractor shall communicate with the Contracting Officer on all matters pertaining to contract administration. Only the Contracting Officer is authorized to make commitments and/or issue changes that will affect price, quantity or quality of performance of the contract. In the event the Contractor effects any such change at the direction of any person other than the Contracting Officer, the change shall be considered to have been made without authority and no adjustment will be made in the contract price to cover any increase in costs incurred as a result thereof. All changes to the contract will be issued via an amendment and/or modifications in writing from the Contracting Officer to the Contractor. Invoices: Invoices are to be submitted electronically; in arrears of the start date of the services that were performed. The Department of Veterans Affairs utilizes a third party invoicing service provider (Tungsten Network) for handling electronic invoices. All invoices should be electronically submitted to the Tungsten Network website: http://www.tungsten-network.com/us/en/. Electronic invoice can be submitted at no additional cost to the Department of Veterans Affairs Vendors. If NOT registered with the Tungsten Network (formerly OB 10), please call (1-877-489-6135) for registration and/or assistance. Work Stoppage/Delays: The Contracting Officer shall be notified at the earliest time available, but, no later than the next business day, via phone of the Contractor's knowledge of any conditions which may adversely affect the Contractor's ability to perform under the terms and conditions as stated in the specifications of this requirement. For example, vehicular problems, licenses, or anything that might jeopardize the terms and conditions of the contract should be reported to the Contracting Officer immediately. TASK SPECIFICATIONS Description of Work: The Contractor shall provide qualified inspectors, labor, technicians, equipment, tools, materials, transportation and maintenance. All labor and material for dish machine and conveyor maintenance to Include -Check for proper operation of Champion dish machine. Note or adjust as required. -Disassemble final rinse nozzles to clean and or replace as needed -Check for mineral build-up and de-lime as required -Inspect for leaks. Note or repair as required -Check for proper temperature and adjust as required -Replace pump seals at 10 months of operation -Check chain drive and chain for proper operation -Check conveyor for loose fasteners/leaks -Check hose reel for leaks -Clean vent at blower/dryer -Reseal joints as needed, supervision, and any other items, and non-personal services deemed necessary to perform. Service Location: Department of Veterans Affairs Loma Linda Healthcare System 11201 Benton St. Loma Linda, CA 92357 Service Requirements: The Contractor shall be responsible for providing the personnel for all components of the annual maintenance and safety inspection for the Department of Veterans Affairs Loma Linda Healthcare System. General Services Information: The Department of Veterans Affairs Hours of Operations: Regular hours: Monday - Friday, 08:00 a. m. to 4:30 p. m. After Hours: Not deemed necessary at this time. If needed, this must be coordinated with the Designated Point of Contact and notify the Contracting Officer. Contractor shall not provide any form of repair services and/or respond to any repair service calls for maintenance. All equipment shall be restored to full service; immediately following completion of the Inspection ACCESS BADGING AND PARKING The Designated Point of Contact is responsible for obtaining access badges and securing parking for Contractors, in accordance with the Department of Veterans Affair Loma Linda Healthcare System Police Department. DELIVERABLES - Contractor shall provide a complete report and/or documented forms of all test results derived from the inspection 5. GOVERNMENT FURNISHED EQUIPMENT AND SERVICES - The Contractor may use the government utilizes (electricity, water) necessary for the performance of services described herein. The Contractor will not have access to the Department of Veterans Affairs Loma Linda Healthcare System information system or access to government computers to access online information while in performance of the services described in the Statement of Work.
