Description
This is a REQUEST FOR INFORMATION (RFI) only pertaining to brand name or equal item(s) for the Northern Indiana VA healthcare facility. Information collected during this RFI may be used in a set aside. If a solicitation is issued, the Government will do so in accordance with Federal Acquisition Circular (FAC) 2005-83. The North American Industry Classification System (NAICS) number is 333241. The NAICS size is 500 employees. Any contractor that believes they are capable and desires to claim preference for small business status must be registered with the SBA at http://web.sba.gov/pro-net/ and meet the requirements of FAR 19.102. Any contractor that believes they are capable and desires to claim preference for veteran owned small business status must be registered with the VIP at https://www.vip.vetbiz.gov as an SDVOSB or VOSB. A Contractor that proposes to provide a supply that is not manufactured by their organization (nonmanufacturer) shall meet the requirements of FAR 19.102(f). Contractors that deem themselves capable of meeting the requirement shall provide the below information to Michael Iffland, Contract Specialist, at michael.iffland@va.gov no-later-than Tuesday, May 23, 2017 at 5:00 PM, EDT. Responses shall include: (1) Business Name and Address, (2) GSA Contract Number, if applicable (3) Point of Contact Name, Phone Number and E-mail Address (4) DUNS and NAICS code (5) Business Size SMALL or LARGE (6) Type of Business: service disabled veteran owned, veteran owned small business, 8a, HUBZone, woman-owned, etc. (7) A concern that is not a manufacturer of the supply shall provide the manufacturers name and size standard. Contractor must be registered with https://www.sam.gov Supplies are to include only TAA compliant items. Items that are not TAA compliant must be clearly identified by the Contractor should the contractor choose to offer a quote if a RFQ is published. FOB is Destination. Delivery is needed 30 days ARO. Delivery is to: VA Northern Indiana Health Care System Fort Wayne Campus 2121 Lake Ave. Fort Wayne, IN 46805-5100 This requirement consists of four (4) line items: ENTREE COUNTER Qty: 1 Nichols Custom Stainless Model No. EC 13632 The counter measures per site dimensions; the top is constructed of 14 gauge stainless steel and the body is constructed out of 18 gauge stainless steel. The counter sits on 6" high x 1 5/8" outside diameter x.065 inches thick stainless steel tubular legs which will be fitted with adjustable stainless steel bullet type feet. Counter includes mounting for Wells MOD 500TD hot well and 125 AMP load center. Mounted to counter to be double overshelves. Overshelves measure approximately 132 long x 18 POD TRAY ASSEMBLY COUNTER Qty: 1 Nichols Custom Stainless Model No. PTA 8134 The pod tray assembly counter measures per site dimensions; the top is constructed of 14 gauge stainless steel and the body is constructed out of 18 gauge stainless steel. The counter sits on 6" high x 1 5/8" outside diameter x.065 inches thick stainless steel tubular legs which will be fitted with adjustable stainless steel bullet type feet. Counter includes (12) tray slides mounted in base of counter (one side only). Mounted to top to be single sided condiment holder with 12 wide overshelf. BEVERAGE TABLE Qty: 1 Nichols Custom Stainless Model No. BT 3032 Beverage table measures per site dimensions. The top and 6 high backsplash are 14 gauge stainless steel with three sides turned straight down 1 1/2" and back 1/2" at 60 degrees from vertical. The base of the table is fully open and supported by 1 5/8" O.D. x.065 thick stainless steel tubular legs fitted with stainless steel leg sockets and adjustable stainless steel bullet feet. The stainless steel shelf is continuously welded to the legs at approximately 10" off the finished floor with all four sides turned straight down 1 1/2" and back 1/2" at 60 degrees from vertical. AIR CURTAIN REFRIGERATOR Qty: 1 FWE / Food Warming Equipment Co., Inc. Model No. R AS 10 Air Curtain Refrigerator, mobile, bottom mount self-contained refrigeration, (20) 14" x 18" or (10) 18" x 26" tray capacity, 4 1/2" OC, removable fixed slides, adjustable thermostat, maintains temperature for up to 1 1/2 hours with door open, digital display, (1) hinged door, full perimeter bumper, stainless steel construction, 6" casters, (2) rigid, (2) swivel with brake, 1/2 HP, UL, cUL, NSF 1 ea Two year limited parts & one year labor warranty, standard 1 ea 120v/50/60/1 ph, 14.7 amps, 1764 watts, NEMA 5 20P, standard (US We are looking for Brand Name or equal that has the Salient Characteristics listed below Salient Characteristics The VA Northern Indiana Health care system has a need for a Tray Line System at its Marion Facility with the following specifications: Pod Tray Assembly Counter 14 gauge stainless steel for top shelving unit 18 gauge stainless steel for base Base needs to be 6 inch off the floor with stainless steel tubular legs The base needs to include a minimum of 8-12 tray slides mounted with 6 in between the trays. The trays that will sit in them measure 18 x 15. Base needs to be 75-80 inches in length and allow for 3 trays that measure 13 x 21 to blank to fit on them at one time. The width of the counter top on the top of the base needs to be a minimum of 24 inches wide x 80 inches in length The top shelving unit needs to be 12-13 inches from the top of the base and have slanted shelving unit that has room for 6 to 8 third size pans to use for condiments. Above the slanted shelving unit needs to be another shelf parallel to the ground approx. 6-7 inches higher. On the right end of the unit needs to have power for the air curtain 120V/50/60/1ph, 14.7 amps The top counter of the base needs to be 35.5-36.5 inches from the floor. Over shelves for Hot Wells The shelves need to have mounting that can attach to Duke s Hot Wells model number E304-25SSM. The hot wells are 60 in long x 24 ¾ inches wide. The bottom shelf needs to be 14 in high from the base of the hot well and the 2nd shelf needs to be a total of 26 inches high from the base of the hot wells. It needs be constructed of 14 gauges stainless steel and the body constructed of 18 gauge stainless steel. The over shelves need to measure approx. 60 in long x 18 inches wide Air Curtain Mobile refrigerator air curtain 20- 12x18 or 10- 18x26 tray capacity Maintains temp for up to 1-1/2 hours with door open Digital display 1 hinged glass door Stainless steel Swivel casters with brakes Minimum of 2 year parts warranty and 1 year labor warranty 120v/50/60/1ph, 14.7 amps, 1764 watts
