This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. The solicitation number is VA256-17-Q-0733 and is issued as an (RFQ) Request for Quotation. The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular FAC 2005-88. This is a total Small Business set-aside. The associated North American Industrial Classification System (NAICS) code for this procurement is 334510 ELECTROMEDICAL AND ELECTROTHERAPEUTI C APPARATUS MANUFACTURING, small business size standard 1250. The units are located in the operating rooms area located at Michael E. DeBakey VA Medical Center, 2002 Holcombe Blvd. Houston TX 77030. Contractor will sign in at Biomed (BA200) and will be escorted to the OR. All repairs must be performed by manufacturer licensed professionals using only original equipment manufacturer parts. Following the repair, vendor must demonstrate to a MEDVAMC Biomed Technician that the equipment runs properly according to manufacturer specified levels. The contract price shall be firm-fixed, all-inclusive, and not limited to specified products/equipment, installation, and setup/configuration. All companies must provide documentation proving that their technicians are certified from the original manufacturer of the equipment to provide service to the equipment specified above. All parts must be certified new from the original manufacturer. DESCRIPTION OF WORK PRIVACY/SECURITY   Contractor will provide and install the Core PC Workstation (p/n 410-3600.08) for Volcano Mobile Unit serial # 121060350001. Any hard drives in the old system must be turned over to the VA for proper disposal Contractor will provide and install CA DVDI-D MALE TO MALE 3 METERS (p/n 806391001) and PC VIDEO DVI-D, 2 CH DA KIT (p/n 420-3104.04) for Volcano s5i serial # 102160294001. Contractor will be contacted to verify and document that the Volcano Systems are working properly and communicate with MEDVAMC staff when the unit is ready for use. FSRs are due within 24 hours of repair to the biomedical engineering department representative. Contractor will coordinate with MEDVAMC staff to ensure no impact to patient care during repair. Contractor will provide the necessary labor to fully complete the job as necessary. The contractor will not have electronic connectivity to the devices and will not have access to patient databases or receive screenshots with PHI present. Delivery - 30 days after receipt of order.
