Description
The Amarillo VA Health Care System (AVAHCS) located in Amarillo, Texas, is seeking Service Disabled Veteran Owned Small Business (SDVOSB) or Veteran Owned Small Business (VOSB) sources capable of meeting the requirement listed below. The acquisition will be accomplished using commercial item procedures in accordance with FAR Part 12. NOTE: TO QUALIFY SDVOSB AND VOSB VENDORS MUST BE CERTIFIED IN THE VA VENDOR INFORMATION PAGES (VIP) DATABASE. The North American Industry Classification System (NAICS) code is 562112. Any SDVOSB or VOSB firms who wish to identify their interests and capability to provide this product must provide product specifications, performance and delivery information by notifying the Contract Specialist no later than 12:00 PM Central Time, May 24, 2017. Notification shall be e-mailed to Robyn Nussbaum, at robyn.nussbaum@va.gov. In accordance with the Federal Acquisition Regulation, responses to this notice are not offers and cannot be accepted by the Government to form a binding contract. Responders are solely responsible for all expenses associated with responding to this Notice. SCOPE: 1. GENERAL SERVICES: Contractor shall provide regulated medical waste services to include reusable sharps containers of specific sizes consistent with end-user and general collection needs; services to pick up sharps, biohazardous waste, chemotherapy waste, and pathology waste at specific intervals for various locations; legal transport, destruction, treatment, and disposition of contents; provision of weights of waste at pick up by type; and manifests to document the cradle-to-grave for the generated waste from the Amarillo VA Health Care System at the following four locations: Amarillo, TX: 6010 Amarillo Boulevard West, Amarillo, TX 79106, (806) 355-9703 Lubbock, TX: 6104 Avenue Q South Drive, Lubbock, TX 79412, (806) 472-3400 Clovis, NM: 921 East Llano Estacado, Clovis, NM 88101, (575) 763-4335 Dalhart, TX: 325 Denver Ave., Dalhart, TX 79022, (806) 249-0673 b. Contractor shall ensure that its staff who drive onto VA property are professional, comply with facility policies including zero tolerance for sexual harassment and violence in the workplace, drug-free workplace, and safety; and engage only authorized staff to sign manifests of waste in accordance with Department of Transportation (DOT) and state requirements. 2. SPECIFIC SERVICES: Provide and Service Reusable Sharps Containment System: Contractor shall provide services including a reusable sharps containment system compliant with CDC and NIOSH guidelines (see http://www.cdc.gov/Niosh/sharps2.html and http://www.cdc.gov/Niosh/sharps3.html,) and which has been approved by the Amarillo VA Health Care System with input from employees that are end users. Contractor shall provide appropriate sizes and shapes, opening types and closures, mounting brackets, carts, training, and replacement for sharps collection and containment, as well as all labor, tools, materials, supplies, appropriate equipment, transportation, and supervision necessary to pick up, store, transport, decant, and clean containers as well as destroy, dispose of, document, and invoice for waste removed from containers. The contractor shall provide these services for each of the four locations identified. The size/count of installed containers for each of the four locations is established in Appendix A. Provide Regulated Medical Waste (RMW) Pickup through Disposal Services: Contractor shall provide services including the provision of containers and labels for regulated medical waste, chemotherapy waste, and pathological waste; pickup of all regulated medical waste streams (sharps, RMW, residual chemotherapy waste per RCRA, and pathological waste); storage and transport; decanting, examination, appropriate treatment, incineration, disposal, and documentation of quantities and weights of waste; cleaning of containers; and provision of legal manifest which accurately documents the chain of custody from point of pickup to final disposition. The contractor shall provide these services for each of the four locations identified. The quantity of waste picked up for each of the four locations is established in Appendix A. Provide Legal Manifests and Documentation: Contractor shall provide timely, accurate, and clear manifests which document quantities of containers and weight of each type of waste picked up at each of the four locations. The manifests must be provided only to authorized representatives to assure COR receives manifest and accompanying documentation. The manifests must meet all federal and state requirements for these regulated medical waste streams. Provide Description of Services and Contingency Plans: Contractor shall provide a detailed description of services provided, including systems in place to assure provision of services in case of system failure for each process.
