- Remit for Payment information for award winners Delivery Information. Please note: All Frozen foods require two separate deliveries. Please review the requirement on the RFQ and on the combined solicitation and synopsis. Request for Quote Document must be filled out in its entirity Combined Solicitation and Synopsis This is a Combined Synopsis/Solicitation for commercial items prepared in accordance with the format in Subpart 12.6 as supplemented with additional information included in this notice. This announcement constitutes the only solicitation of offers that are being requested. The solicitation number is RFQP02101700016 and this solicitation is issued as a Request For Quote (RFQ). PLEASE READ THROUGH ITS ENTIRITY. Solicitation is for 4th Quarter Subsistence, FY 2017 (Grocer, Frozen and Meat Items). For a list, description and total quantities of all items, refer to Section 2 Commodity or Services Schedule. Solicitation is a 100% small business set aside. The NAICS for this requirement is 311999. Please organize the address of delivery and time with the Food Services Department at the contract information below. Terms of delivery are F.O.B. destination to: Building 4 181 Flushing Avenue Brooklyn, NY 11205 Delivery Hours 8 AM to 3 PM Contact: Rocco Lupo, Food Services Administrator 646-836-6300 x6344 Robert Lee, Warehouse Manager 646-836-6591 Note: All frozen items shall be split into two equal deliveries. The first delivery will occur in July; the second delivery will occur in August. The vendor must call the warehouse to schedule both deliveries. Please quote according to this requirement. The deliveries are expected to be received July 3, 2017 through August 31, 2017. Any items shipped before or after the required delivery time-frame will be returned at vendor's expense. Upon Award, the Food Service Warehouse must be contacted prior to delivery for approval or adjustments in delivery schedule using the information above. Quoter(s) are advised the government reserves the right to make single or multiple awards. The government will award to the responsible quoter(s) whose quote, is conforming to the solicitation. This request for quote may be awarded as an all or none award. The aggregate total of all line items may be used to determine the award for this RFQ. Evaluation Factors are past performance, price, and delivery. Past performance is significantly more important than price and non-price related factors. Award will be based on the best value to the government considering the evaluation factors listed above. Best value may be determined using tradeoffs among the evaluation factors, so that award may be made to other than the lowest-priced offer. Vendors who submit an offer concurs that delivery date can be met, unless otherwise noted. Products other than canned goods must be 3 months or younger (canned good must be 1 year or younger) upon delivery with the date (including expiration date, best by date, sell by date, use by date or consumer by date) on the label unless otherwise noted. No stamped/written dates allowed. PAST PERFORMANCE RECORD OF "FAILURE TO DELIVER" ON PREVIOUS AWARDS MAY RESULT IN A LESS THAN FAVORABLE PERFORMANCE RATING. Piggy back offers will NOT be evaluated and therefore NOT considered for award. DO NOT CHANGE CASE COUNTS!! YOUR OFFER WILL NOT BE CONSIDERED! SUBSTITUTIONS TO SPECIFICATIONS IN LINE ITEMS ARE PROHIBITED (ex. SIZES, CASE COUNTS). CHANGES TO SPECIFICATIONS WILL NOT BE EVALUTED AND THEREFORE, NOT CONSIDERED FOR AWARD. THE GOVERNMENT RESERVES THE RIGHT UPON INSPECTION TO REJECT ITEMS IF THEY ARE DAMAGED OR NOT TO THE SPECIFICATIONS AS LISTED IN THE SOCLITATION. ALL BIDS MUST MEET SPECIFICATIONS. ALL BIDS MUST BE CLEAR! Do not provide a Unit Price past the 4th decimal point for your quote or that line item will not be considered. For bid consideration for any kosher items offered, follow the Certified Religious Diet Specifications Quote Sheet in this solicitation. All kosher items offered on this bid must be certified by and accepted by the Orthodox Kosher Certification Agency from the United States or Canada. Quoter(s) MUST include a Letter of Certification from the religious certification agency with all quote(s) submitted. Please submit the Kosher Certification(s) (date valid from time of bid submittal through the delivery quarter) with your quote. Follow the Certified Religious Diet Specifications Quote Sheet. The completed package should be returned no later than 8:00 a.m. Eastern Standard Time, May 24, 2017. Vendors shall submit signed and dated offers to Michelle Thomas, Contracting Officer by email to mthomas@bop.gov ONLY. Fax, hand delivery, nor mail-in will be accepted. Only quotes submitted to mthomas@bop.gov will be considered. Be sure to complete blocks 17a, 23, 24, 30a, 30b, and 30c of SF-1449 and the continuation sheets. Please include your TELEPHONE number, e-mail address AND DUNS in block 17a. ALL offers shall be submitted on the specific RFQ/SF1449.
